Every June, India sends its children back to school with new uniforms, polished shoes, sharpened pencils, labelled water bottles and a strange confidence that the academic year begins on a clean slate.

It does not.

In pediatric outpatient departments, the new school year announces itself differently. It arrives as fever. It arrives as cough, cold, wheeze, sore throat, vomiting, fatigue, poor feeding, breathlessness, and that familiar phrase parents bring with them: “Doctor, school has just reopened.”

When Schools Reopen, Viruses Return

Within a few weeks of school reopening, the pattern becomes hard to miss. Daycare children start falling sick. Nursery children follow. Primary school children carry infection back home. Pediatric OPDs swell. Wards begin to fill. Step-down PICUs see more children with viral wheeze, bronchiolitis, influenza-like illness and Pneumonias. Some children recover in two days. Some need oxygen. Some need high-flow support. Some fragile infants and children with underlying disease get pushed into severe illness by viruses that began their journey in crowded classrooms and daycare rooms.

And the chain does not stop with the child…!

This happens every year. We discuss it in OPD corridors, pediatric meetings, parent WhatsApp groups and school offices. We complain about it. We warn each other about it. But we still do not have a simple, widely used, school-facing protocol for the June to September viral surge.

That is the failure.

India has disease surveillance and influenza like illnesses data. India has pediatricians who can recognize the seasonal rise before any dashboard does. India has schools that can mobilize parents overnight for fees, uniforms, transport changes, annual day costumes and examination circulars. Yet when it comes to preventing predictable viral spread after school reopening, most schools still operate on improvisation, denial and attendance pressure.

We are not dealing with one virus. That is the first point schools must understand. The child with fever and cough in July may have influenza, RSV, rhinovirus, adenovirus, human metapneumovirus, parainfluenza, SARS-CoV-2, or a combination of viruses. Clinically, many of these infections look similar at the beginning. A mild cold in one child can become bronchiolitis in an infant, wheeze in an asthmatic child, pneumonia-like illness in another, and hospitalization in a high-risk child.

The second point is more uncomfortable. Schools and daycare centres are perfect transmission machines unless they are deliberately managed. Young children sit close, share toys, touch faces, wipe noses, cough openly, forget hand hygiene, sleep in groups, eat together, cry into caregivers’ shoulders, and often cannot explain symptoms early. Nursery and daycare children cannot be expected to behave like trained adults. Infection control in these spaces cannot depend on the child’s discipline. It has to depend on adult systems.

The third point is that school reopening in India overlaps with the monsoon respiratory-virus season in many regions. This is not just an anecdote from one pediatric OPD. Indian surveillance and hospital studies have repeatedly shown that influenza-like illness and severe acute respiratory infection rise in seasonal patterns, with monsoon and post-monsoon peaks in several parts of the country. The exact virus changes. The geography changes. The timing varies between North India, South India, Western India, Eastern India and the Northeast. But the larger truth remains: June to September is not a neutral period. It is a biologically risky window.

Yet our school calendar behaves as if infection risk is irrelevant….!

We reopen crowded institutions into a known transmission season. We allow febrile children to attend because parents cannot miss work, because exams are near, because daycare is essential, because “it is only a cold,” or because schools quietly reward attendance more than responsibility. Then we wait for pediatric OPDs to absorb the consequences.

This is not a call to close schools. That would be irrational and inappropriate. Children need school. They need learning, food, play, language, friendships, discipline, teachers, routine and escape from the loneliness that screens have created. Repeated closures damage children in ways we have already seen during the pandemic.

The question is not whether schools should remain open. They should.

The question is why they remain open without a respiratory infection protocol…!

A sensible school-health policy does not begin with panic. It begins with thresholds. It tells schools what to do before the outbreak, during the outbreak, and after the outbreak. It tells parents when not to send a child. It tells teachers what to watch for. It tells daycare staff how to reduce spread. It tells pediatricians and public health authorities when a cluster deserves attention.

At the minimum, every daycare, nursery, kindergarten and primary school should have a June to September respiratory illness plan.

The first step is pre-reopening preparation. Schools should send a health advisory before reopening, not after half the class is coughing. Parents should be told clearly that fever, active cough, breathing difficulty, vomiting, loose stools, lethargy or poor feeding are valid reasons to keep a child home. Schools must stop demanding medical certificates for every short illness absence. That single habit drives unnecessary OPD visits, exposes other children in clinics, burdens parents, and turns pediatricians into attendance clerks…!

The second step is vaccination awareness. Influenza vaccination is not a magic shield, but it is a rational preventive tool, especially for children at higher risk of severe illness, children with asthma or chronic lung disease, congenital heart disease, neurodevelopmental disability, immunosuppression, prematurity history, and those living with elderly or medically fragile adults. Staff in daycare and nursery settings should also be part of this conversation. A school cannot claim to care about child health and then ignore preventable influenza morbidity.

The third step is the sick-child rule. It has to be simple enough to print on a school notice board. Fever means stay home. A child with fever should return only after being fever-free for at least 24 hours without antipyretics and clinically better. A child with persistent cough, breathlessness, fast breathing, poor oral intake, drowsiness, chest indrawing, bluish lips, or oxygen saturation concern needs medical evaluation, not classroom sympathy. Toddlers with active respiratory symptoms should not be sent to daycare merely because adults have no backup plan. That is harsh to say, but necessary. Daycare convenience cannot become community transmission policy.

The fourth step is class-level surveillance. Schools already maintain attendance registers. They need one additional column during June to September: reason for absence. Fever. Cough. Cold. Wheeze. Vomiting. Loose stools. Rash. Unknown. If three or more children in one class develop influenza-like symptoms within a few days, the school should alert its medical advisor or local health contact. If absenteeism rises sharply in a class, the school should not wait for rumours. It should act.

The fifth step is layered control. During clusters, schools can reduce assemblies, improve ventilation, avoid indoor crowding, stagger meals, clean high-touch surfaces, disinfect shared toys in daycare, keep symptomatic staff away, and encourage masks for older children and staff during active outbreaks. None of this is dramatic. None of this requires shutting the school; but requires seriousness.

The sixth step is honest parent communication. Schools should stop sending vague circulars saying “viral fever is spreading, take care.” That means nothing. Parents need clear instructions: do not send a febrile child; inform the class teacher if your child has influenza-like illness; seek care urgently for breathing difficulty, poor feeding, persistent high fever, lethargy, seizures, dehydration or worsening wheeze; protect infants and elderly household members if one child is sick. Good communication reduces panic. Bad communication creates gossip.

The seventh step is hospital-public health linkage. Pediatricians are often the first to see the signal. When multiple children from the same school or locality present with similar illness, there should be a simple reporting pathway. Not every cluster needs laboratory testing. Not every outbreak needs media attention. But repeated clustering should not remain trapped as OPD anecdote. It should become surveillance intelligence.

This is where India’s current approach is incomplete. We have ILI and SARI definitions. We have national surveillance networks. We have laboratory capacity in many regions. We have outbreak investigation experience. But these systems often sit above the everyday school setting. Schools need a practical bridge between clinical medicine and public health.

A school respiratory protocol should not be a 90-page document that nobody reads. It should be a two-page operational checklist.

• Before reopening: advisory, vaccination awareness, sick-child rule, ventilation check, daycare cleaning plan.

• During routine weeks: absenteeism tracking, hand hygiene, cough etiquette, symptom-based exclusion, parent communication.

• During clusters: class-level alert, temporary reduction of crowding, masking where feasible, enhanced cleaning, medical consultation, public health intimation if severe or unusual.

• During severe outbreaks: targeted class-level pause only if needed, not blind school closure.

• After the season: review what happened, how many children were absent, how many were hospitalized, what worked, and what failed.

It protects learning by preventing avoidable disruption. It protects children by reducing viral load and repeated exposure. It protects hospitals by flattening seasonal surges. It protects grandparents by recognizing that the school-child-household chain is real.

The opposition to such a protocol will be predictable. Some will say children fall sick anyway. True. Children do fall sick. A young child may have several respiratory infections a year. But inevitability is not the same as helplessness. Road accidents happen too. We still use helmets, seat belts, speed limits and school-zone rules. Viral infections will continue. That is not an argument against prevention. It is an argument for adult competence.

Some will say schools cannot manage this. That is false. Schools already manage far more complex systems when they want to: transport routes, fee collection, examinations, uniform rules, digital portals, parent meetings, CCTV, annual functions and competitive coaching schedules. A fever policy is not beyond institutional capacity. It is beyond institutional priority.

Some will say parents will not cooperate. Some will not. That is exactly why schools need written policy. A parent who sends a febrile child should not be treated as merely “adjusting.” They are placing other children and households at risk. Working parents need support, but the solution cannot be to convert daycare into an infection exchange.

Some will say this is alarmist. It is not. Alarmism is shouting after the ward is full. Public health is acting before the ward fills.

The June to September pediatric viral surge is not a mystery. It is a recurring event hiding in plain sight. Every pediatrician knows it. Every school has seen it. Every parent has lived it. Every hospital has absorbed it. What we lack is the courage to stop calling it normal.

The first lesson of the school year should not be that fever spreads faster than policy.

If we can reopen schools with new timetables, new books and new fee structures, we can reopen them with a respiratory illness protocol. The children are already giving us the data. They give it through crowded OPDs, full pediatric wards, anxious parents, exhausted nurses and oxygen beds occupied by illnesses that began as “just a cold” in a classroom.

Every June, the virus enters school with the children.

The question is whether adults will finally be ready before it does…..!

I did not ask immediately.

In medicine, some silences arrive already diagnosed.

After a few moments, he told me the story. His biology professor from his pre-university college days had called him from far away. He was 65 years old now. A year earlier, this man had still been teaching biology to pre-university students. He was working, explaining life to young minds, using his voice, his hands, his memory, his body, and his years of accumulated confidence in the classroom. Then came an illness that began as weakness and later acquired a name: Guillain-Barré syndrome.

The name is tidy. The disease is not…!

On paper, Guillain-Barré syndrome is an acute immune-mediated neuropathy, often triggered after an infection, in which the body’s immune system attacks peripheral nerves. In real life, it can be a terrifying betrayal. A person who was walking, speaking, swallowing, eating, teaching and breathing normally may, over days, begin to lose the ordinary functions that make life feel secure. Legs fail. Hands weaken. Voice changes. Swallowing becomes unsafe. Breathing, that quiet background rhythm of existence, becomes an excessive work...!

We speak of “ascending paralysis” in textbooks. But no family experiences it as a phrase. They experience it as panic moving upward.

The family first took him to a local hospital. The doctor there assessed him and referred him to a tertiary care hospital in Bengaluru. The patient was shifted by road in an ambulance. Somewhere on the way, near Chitradurga, his condition worsened. He was gasping. A doctor in Bengaluru, guiding them over the phone, advised them to stop at a nearby teaching medical college hospital. He was stabilized there for a few hours. Then the family continued the journey and finally reached a corporate tertiary care hospital in Bengaluru.

That journey took more than forty hours…!

Forty hours is not a number when a nerve disease is moving toward the lungs. Forty hours is not distance on a highway. It is biology (pathology) unfolding without permission. It is oxygen, panic, traffic, waiting, pleading, calling, guessing, hoping. It is a family sitting beside a deteriorating teacher, trying to understand whether they are moving toward safety or merely moving.

In the months (to be precise 13 months) that followed, he went through tracheostomy, cardiac arrests, prolonged intensive care, multiple sessions of plasmapheresis, supportive treatment, nursing care, physiotherapy and a year of uncertainty that no discharge summary can contain. His body fought. Medicine fought. His family fought. There must have been days when one vital sign felt like victory, and days when a small setback must have felt like betrayal.

He survived.

That sentence deserves respect.

He survived because modern medicine exists. He survived because ventilators exist. He survived because intensive care exists. He survived because plasmapheresis exists. He survived because tracheostomy, monitoring, antibiotics, suctioning, nutrition, physiotherapy and rehabilitation exist. He survived because nurses, intensivists, neurologists, physiotherapists, technicians, family members and invisible hands kept him alive when his own body had almost withdrawn cooperation.

It is fashionable now to speak of hospitals, doctors, pharma and modern medicine only through suspicion. Some suspicion is earned. Healthcare has commercial excess, institutional arrogance, poor communication, avoidable delay and ethical blindness. But criticism becomes foolish when it forgets the obvious: a 65-year-old man with severe Guillain-Barré syndrome, respiratory failure, cardiac arrests and prolonged ICU dependence surviving for a year is not a small thing. It is a triumph of science, skill, endurance and human labour…!

Modern medicine did not fail this man. It pulled him back from death.

But survival is not the same as return.

Today, after one year, he is wheelchair-bound. He has vocal cord palsy. He has lost mobility. His life has been rearranged by one illness. His family’s life has also been rearranged. In chronic neurological disability, the patient is not the only patient. The family becomes an unpaid ICU, an informal rehabilitation centre, a financial shock absorber, a nursing unit, a transport service, a rehabilitation coordinator, and often, silently, a casualty.

Someone must feed. Someone must lift. Someone must clean. Someone must arrange physiotherapy. Someone must understand prescriptions. Someone must call hospitals. Someone must borrow money. Someone must stop working. Someone must sleep lightly, because the patient may need help at night. Families do not merely “support” such patients. They are absorbed into the illness.

This is what disturbed my colleague. And this is what disturbed me…!

Not because Guillain-Barré syndrome is always preventable. It is not. Not because every patient with GBS can be brought back fully. They cannot. Not because every bad outcome is someone’s fault. That is immature thinking. Disease has its own violence. Biology is not obedient. The human body has limits, and medicine, even at its best, does not erase them.

But some cases do not allow us to hide behind disease alone.

• What if the first recognition had been sharper?

• What if respiratory weakness had been anticipated earlier?

• What if the ambulance had been equipped to support a deteriorating neurological patient, rather than function mainly as a vehicle with a siren?

• What if the receiving hospital had been alerted before transfer?

• What if the referral had been not just to a bigger place, but to the right place, at the right time, with stabilization and handover?

• What if a region with so many hospitals, medical colleges and corporate centres had a referral network humane enough that a frightened family did not have to depend on a stranger’s voice on the phone to find its way?

These questions are uncomfortable because they produce no clean villain.

The problem is not one doctor, one hospital or one ambulance. The problem is a system in which responsibility becomes blurry exactly when clarity is needed most. Everyone does a part. Nobody owns the whole journey. The patient moves from one institution to another, but the illness does not wait for paperwork, bed availability, geography, institutional ego or the family’s ability to negotiate.

This is one of the great paradoxes of modern healthcare. We have ventilators, but not always timely ventilation. We have neurologists, but not always reachable ones. We have plasmapheresis, but not always where the patient first needs it. We have hospitals everywhere, but not always a system that connects them intelligently.

The body is fragile. That is the first truth.

Most days we forget this. The body obeys so quietly that we mistake its obedience for simplicity. We walk, speak, eat, sleep, teach, drive, write, swallow and breathe without gratitude. Then one immune-mediated storm attacks the peripheral nerves and the fiction collapses. Suddenly, every ordinary act reveals itself as a sophisticated biological achievement. Breathing is not simple. Swallowing is not simple. Standing is not simple. Voice is not simple. Life is not simple. It only appears simple when the body is kind.

The second truth is that modern medicine is astonishing.

A century ago, many patients with severe neuromuscular respiratory failure would simply have died. Today, a patient can be ventilated, fed, monitored, protected from infection, treated with plasma exchange or immunoglobulin, rehabilitated and kept alive through a storm that would once have been fatal. This is not sentiment. This is science turned into infrastructure. A ventilator is not just a machine. It is physiology made mechanical. An ICU is not just a room. It is organized vigilance.

But the third truth is harsher.

Modern medicine can save a life, while the healthcare system wounds the same life through delay, confusion, poor triage and abandonment disguised as referral…!

This is where the ethical question begins.

Medical ethics is not only consent forms, confidentiality and signatures. Those matter, but they are not enough. In an emergency, ethics lives in recognition, communication, stabilization, transfer and ownership.

• Do we recognize severity early?

• Do we tell the family the truth without either panic or false reassurance?

• Do we stabilize before sending?

• Do we call ahead? Do we document clearly?

• Do we transfer to the correct level of care? Do we make sure the ambulance can handle what may happen on the road?

Referral is not clerical work. Referral is a clinical intervention…!

Done well, it saves. Done badly, it becomes risk displacement. A patient is moved, but responsibility is not. A name is written, a destination is suggested, an ambulance leaves, and everyone feels that something has been done. But the patient’s biology continues in real time. In Guillain-Barré syndrome, time does not behave like a calendar. It behaves like nerve conduction, respiratory muscle strength, swallowing safety and autonomic instability. A few hours may mean little in one patient and everything in another.

This case also exposes a middle-class (in particular) illusion about healthcare: that Grand looking hospitals mean healthcare exists….

They do not.

A city may have medical colleges, private hospitals, specialists, diagnostic centres and ICUs, and a patient may still be functionally alone inside that abundance. Healthcare is not the presence of buildings. It is the reliable movement of the right patient to the right care at the right time, without forcing the family to become logisticians during a medical emergency.

We see versions of this all the time. A child with sepsis. A pregnant woman with eclampsia. A stroke patient. A newborn with respiratory distress. A teacher with ascending paralysis. The story changes, but the pattern is familiar. The disease begins. The family searches. The first facility hesitates or refers. The ambulance is uncertain. The receiving centre is not ready. The definitive care comes late. Science rescues what it can. The family pays for what the system did not organize. Later, everyone calls it fate.

Some of it may be fate.

But not all of it.

That distinction matters. If we call everything negligence, we become unfair and stupid. If we call everything fate, we become morally useless. The truth is harder. Some damage comes from disease. Some comes from delay. Some comes from the randomness of access. Some comes from the fact that healthcare is still designed around institutions rather than around the patient’s journey through them.

What should disturb us is not only that such stories happen. What should disturb us is that we have grown used to them. We discuss them in corridors, shake our heads, praise the final centre, pity the family and move on. We have blame culture when something becomes public, and silence culture when it does not. What we do not have enough of is just culture: the discipline to examine a bad journey honestly, without immediately hunting for a scapegoat or protecting reputation.

A serious healthcare system would study such journeys.

• Where was severity first recognized?

• Was respiratory function assessed?

• Was the transfer appropriate?

• Was the ambulance equipped?

• Was the receiving unit informed?

• Was the family counselled?

• Could the patient have reached definitive care earlier?

Not to punish blindly. Not to shame individuals. But to stop pretending that a patient’s path through the system is someone else’s problem.

This one case contains everything: the fragility of the human body, the power of modern medicine, the exhaustion of families, the ethics of referral, and the unfinished accountability of our healthcare system.

The professor survived. His family survived in another way. My colleague was shaken because he knew the patient not as a bed number, but as a teacher from his own past, a human being whose life had been permanently altered. I was shaken because the story felt painfully familiar. Every doctor has seen versions of it. Every family fears becoming the next version.

There are cases that teach diagnosis. There are cases that teach humility. There are cases that expose the system.

This was all three.

Kudos to the patient and his family for enduring what no family should have to endure. Kudos to modern medicine for pulling him back from death. Kudos to every nurse, doctor, technician, physiotherapist and caregiver who helped keep him alive through that long year.

But shame on us if we stop at admiration…!

The correct response to such a story is not sympathy alone. It is referral discipline. It is early recognition. It is equipped transfer. It is accountable handover. It is regional coordination. It is the refusal to let another family travel forty hours through uncertainty while a treatable neurological emergency climbs silently toward the lungs.

A human body can fail without warning.

A healthcare system should not…!

Cardiometabolic multiple long-term conditions (MLTC), defined as the coexistence of two or more cardiometabolic diseases such as diabetes, cardiovascular disease, and chronic kidney disease, are increasingly prevalent and represent a growing challenge for health systems worldwide.

Evolution of terminology of multimorbidity

•The term multimorbidity was first described by Brandlmeier in an urban German general practice in 1976.

•In view of the confusion in terminologies between comorbidity and multimorbidity, a distinction was proposed in 1996 with the term comorbidity implying combined effects in reference to an index chronic condition (eg, stroke in a person with diabetes) and multimorbidity meaning co-occurrence of conditions without a single condition holding a priority over other co-occuring conditions.

•The concept of multimorbidity as two or more chronic conditions was adopted by WHO in 2008 in recognition of its growing burden in an ageing society.

•In 2018, the Academy of Medical Sciences published a landmark international policy report titled Multimorbidity: a priority for global health research. The report highlighted the rising burden of multiple long-term conditions and the lack of coherent evidence on its causes, prevention, and management.

•Patients do not find multimorbidity an acceptable term to describe their condition and associate it with a negative image, giving the impression to patients that they are in poor health, their body is failing them, they may be dying soon, and there is nothing they can do about their condition. The UK National Institute for Health and Care Research Strategic Framework therefore emphasised their preferred term as multiple long-term conditions.

Determinants of cardiometabolic MLTC

Cardiometabolic MLTC arise from a complex interplay of biological, behavioural, social, and environmental determinants; genetic susceptibility; metabolic pathways; ageing; adiposity; insulin resistance; and chronic inflammation, with some determinants being modifiable . One genome-wide association study showed that there are 11 independent genetic variants associated with psycho-cardiometabolic MLTC, with a polygenic risk score being predictive of co-occurrence of depression, coronary artery disease, and type 2 diabetes, and suggested eight causal factors (BMI, body fat percentage, LDL cholesterol, total cholesterol, fasting insulin, income, insomnia, and childhood maltreatment).

Population-level interventions

•High-quality evaluations using real-world data, rather than modelling studies, are needed for assessing the effect of population-level interventions on the prevention and management of cardiometabolic multiple long-term conditions (MLTC).

•There is limited evidence on populations at high risk, such as those from socioeconomically deprived areas and minority ethnic communities.

•Most studies have been in older populations and further research is needed for younger populations who are facing a growing burden of MLTC.

•There is a scarcity of high-quality trials in low-income and middle-income countries.

Individual-level interventions

•Clarity is required as to what are the most meaningful primary endpoints for intervention trials in individuals with, or at risk of, cardiometabolic MLTC.

•There is insufficient high-quality evidence that in-person behavioural interventions in people with cardiometabolic MLTC produce sustained improvements in risk factor control and cardiometabolic outcomes.

•Evidence for digital and artificial intelligence-based delivery of lifestyle interventions to improve clinical outcomes and address health inequalities remains scarce.

•Evidence is needed to understand optimal timing and intensity of treatment with novel pharmacotherapies in different risk groups across the life course in cardiometabolic MLTC.

•Further evidence is required to understand the effect of rational polypharmacy and deprescribing on clinical outcomes in cardiometabolic MLTC across the life course.

System-level interventions

•Further research is required on pragmatic models of care for improving outcomes of people with established cardiometabolic MLTC.

•There is a lack of multimorbidity-specific outcomes, with most studies prioritising single behaviours or conditions rather than outcomes relevant to MLTC (eg, treatment burden, functional status, or integrated risk reduction).

•Future studies should include complex interventions integrating behavioural interventions, digital tools, care coordination, and health system infrastructure.

•Evidence for system-level digital interventions, including artificial intelligence and large language model-enabled decision-support systems, remains scarce.

He had just had a baby. The delivery happened in a famous hospital. The mother was fine. The baby was fine. The rooms were good. The staff were polite. The process was smooth. The family was reassured. The bill was around five lakh rupees.

Then he asked me, “Did my spending five lakhs for my child’s birth assure me of something more than a private delivery in a government hospital for free? Did it change any outcome?”

I did not have a clean answer…!

That embarrassed me a little, because I work in a medical college. I Head the Pediatric department. I sit in academic sessions, mortality meetings, morbidity meetings, briefings, debriefings, audits, departmental reviews, institutional discussions, and all the small rituals through which medical systems try to convince themselves that they are serious about quality. I know many government and teaching hospital teams that are deeply committed, technically strong, underpaid, overworked, and still deliver remarkably good care. In many such places, childbirth happens for free, or at a cost that may be less than what one pays for a family dinner in a large city hotel. Women get reasonable labour support. Newborns get essential care. Emergencies are handled. Complications are referred or managed. Life goes on without marble flooring and valet parking…!

And yet, when this friend asked me whether five lakhs had bought him better medical outcomes, I could not answer with the confidence that an academic should have.

That is the discomfort.

In India, childbirth is never just childbirth. It is biology, family anxiety, social signalling, fear of regret, reputation of the doctor, pressure from relatives, fear of government hospitals, aspiration for privacy, and the deep middle-class desire to say, “We did the best possible thing.” The hospital is not chosen only for its clinical protocols. It is chosen for the feeling it gives the family when everyone is afraid. A polished reception desk, a known obstetrician, a clean room, a nurse who answers quickly, a neonatologist available on call, a lift that works, a bill printed with confidence, all of this becomes part of the experience of safety.

But experience of safety is not the same as safety….!

That is where we confuse hospital service with hospitality….!

Hospitality is not useless. It is not some shallow luxury to be mocked by those of us sitting in teaching hospitals. Privacy matters. Clean toilets matter. Respectful speech matters. A calm labour room matters. A husband or mother being treated with dignity matters. A woman not being shouted at during labour matters. A newborn not being passed around in a chaotic corridor matters. A family receiving clear explanations matters. These are not “extra” things. They are part of humane care.

The problem begins when we assume that because the hospitality is better, the healthcare outcome must also be better.

A five-lakh birthing package may give comfort, privacy, speed, reputation, and emotional reassurance. It may also give excellent medical care. Many private hospitals do. But those are two different claims. One is visible. The other must be measured.

The visible things are easy to sell. Room category, consultant name, attendant bed, food, nursing attention, package inclusions, luxury, convenience.

The invisible things are harder. What is the hospital’s primary caesarean section rate? How often is labour induced? How many babies are admitted to NICU? How many of those admissions are medically necessary? What is the rate of postpartum hemorrhage? How many mothers need ICU care? How often are antibiotics used? How early is breastfeeding started? How many babies are readmitted with jaundice, sepsis, or feeding problems? What is the episiotomy rate? How many women report respectful care? How many felt coerced? How many were actually offered a trial of labour when appropriate?

These are not abstract academic questions. These are the questions a family should be able to ask before spending five lakhs.

But in the Indian birthing market, families rarely get such answers. They get reputation. They get word of mouth. They get WhatsApp recommendations. They get Google reviews. They get the name of a famous doctor. They get the comfort of a known brand. They get a package estimate. They get fear converted into a purchasable plan.

And maybe that is enough for many families. Maybe they do not want an outcome dashboard. Maybe at the time of childbirth, nobody wants to read hospital statistics. They want the mother and baby to come home safely. They want someone competent to be present if things go wrong. They want dignity. They want predictability. They want not to be trapped in the chaos that many Indians associate with public hospitals.

That fear is real. It cannot be dismissed.

Government and teaching hospitals may have clinical strength, but many of them still fail the dignity test. They may manage hemorrhage well but communicate badly. They may save a baby but humiliate the family. They may have good doctors but broken systems. They may have experience with complications but no time to explain. A poor woman is expected to tolerate this because she has no choice. A middle-class family pays to escape it. A rich family pays to never even see it.

So the moral answer is not simple…!

It is not enough to say that expensive hospitals are exploiting people. Some are. But some are also filling the gaps that public systems have left open for decades: dignity, comfort, privacy, communication, and the feeling that the family is seen.

It is also not enough to say that expensive care is automatically better care. That is a ridiculous assumption. In medicine, price is not a biomarker of quality. A large bill does not prove better outcomes. A famous name does not prove rational care. A suite room does not prove safer labour. A polished discharge summary does not prove better newborn follow-up.

This is where academia should help. But does it?

I searched for clear academic answers. I looked for literature that could help me respond to my friend in a responsible way. I found many articles on maternal health, institutional delivery, caesarean rates, public-private differences, respectful maternity care, out-of-pocket expenditure, and health system inequity. But I did not find the kind of answer an ordinary Indian family actually needs.

Not a theoretical answer. Not a moral answer. Not a slogan.

A practical answer.

“If I spend five lakhs for childbirth in a famous hospital, what measurable advantage am I buying over a competent public or teaching hospital?”

That answer is still missing.

And this absence itself says something about us.

We produce academic work. We conduct meetings. We discuss audits. We speak about quality. We publish papers. Many teaching hospitals publish more than several corporate hospitals. Yet when a family asks a basic question about the value of an expensive birth, our system becomes strangely vague. We have data in fragments, but not in a form that helps citizens compare care. We have claims, but not enough public accountability. We have reputation, but not enough transparency…!

In a serious health system, hospitals offering childbirth care should be able to publish meaningful outcome indicators in a way families can understand. Not as marketing brochures. Not as selective success stories. Not as photographs of smiling mothers and babies. Real numbers. Caesarean rates. NICU admission rates. Maternal complications. Neonatal outcomes. Breastfeeding support. Readmissions. Infection rates. Patient-reported dignity. And these numbers should be risk-adjusted, because a referral hospital seeing high-risk pregnancies cannot be compared crudely with a centre handling mostly low-risk deliveries.

This is not to shame hospitals. It is to make trust measurable…!

Because trust without data becomes branding….

My friend was not asking whether his hospital was good. His wife and child were safe, and that matters more than any argument. He was asking something more difficult: whether the extra money bought better medicine, or mainly a better experience around medicine.

I still do not know.

Maybe he bought faster access. Maybe he bought a lower threshold for escalation. Maybe he bought the assurance that a senior doctor would appear quickly. Maybe he bought privacy and dignity. Maybe he bought protection from the uncertainty and crowding of an overstretched public system. Maybe he bought an atmosphere where his family felt looked after. Maybe he bought hospitality. Maybe he bought healthcare. Most likely, he bought a mixture of both.

But I cannot honestly tell him how much of each.

And that is where my reflection ends, not with an answer, but with a discomfort I cannot shake off. For all the academia I have around me, for all the meetings, audits, reviews, journal articles, mortality discussions, and institutional vocabulary of quality, I still do not have a credible answer to one intelligent father’s question: when an Indian family pays five lakhs for a birth, what exactly has improved?

Until we can answer that clearly, families will continue to judge hospitals the way they judge hotels: by reputation, comfort, polish, and fear. And childbirth, one of the most human moments in medicine, will remain trapped between safety and status, between care and comfort, between hospital service and hospitality.

Variables are the building blocks of medical research. They define what we are studying, what we are measuring, what may be influencing the result, and what outcome finally matters to patients. Without clear variables, research becomes vague, analysis becomes weak, and conclusions become unsafe.

In healthcare, variables are not just statistical labels. Age, severity of illness, nutritional status, socioeconomic background, timing of treatment, laboratory values, and clinical outcomes all shape real decisions. They decide whether a finding applies to a patient, whether an intervention is useful, whether a risk factor is truly causal, and whether a policy will work in the real world.

In health delivery, good variable thinking helps us move from data to action. It tells us whom to screen, whom to treat, what to monitor, what to control for, and where systems are failing.

A doctor, researcher, teacher, or policymaker who understands variables is less likely to be misled by associations and more likely to make decisions that are fair, precise, and clinically meaningful.

1. The simplest way to think about variables

The medical research question always has this skeleton

Exposure / Intervention → Outcome

Example:

Early antibiotics in suspected neonatal sepsis → Mortality

But real patients are not clean equations. They bring age, gestation, birth weight, severity, culture status, referral delay, maternal risk factors, prior antibiotics, hospital capacity, and clinician judgment into the picture.

That is why variables matter. They decide whether your conclusion is clinically useful or dangerously misleading….!!

STROBE stresses that observational studies must clearly describe exposures, outcomes, confounders, effect modifiers, and statistical methods because poor reporting makes clinical interpretation weak. CONSORT similarly exists because randomized trials need transparent reporting before readers can judge whether the result is believable.

2. Independent variable

What it is

The independent variable is the exposure, risk factor, treatment, test, or intervention whose effect you are studying.

It answers:

“What are we testing as the possible cause or predictor?”

How it affects decision-making

If you define the independent variable badly, the whole study collapses.

Example:

“Antibiotic use” is vague.

Better:

“Receipt of intravenous antibiotics within 1 hour of sepsis recognition.”

Why? Because clinical decisions depend on precision. A policy-maker cannot act on “antibiotics help.” They need to know which antibiotic, at what timing, for which patient, at what severity, and with what trade-off.

3. Dependent variable

What it is

The dependent variable is the outcome.

It answers:

“What changed?”

How it affects decision-making

A weak outcome produces weak medicine.

Example:

A study shows that a drug reduces fever duration by 6 hours. But it does not reduce hospitalization, complications, antibiotic use, cost, or mortality. That result may be statistically significant, but clinically small.

This is why evidence-based decision-making does not stop at p-values. GRADE asks decision-makers to judge certainty of evidence, balance of benefits and harms, patient values, resources, equity, acceptability, and feasibility before making recommendations.

4. Confounding variable

What it is

A confounder is a third variable that is related to both the exposure and the outcome, creating a false or exaggerated association.

It answers:

“Is something else actually explaining this result?”

Classic medical example

Research question:

Does formula feeding increase infant infections?

Possible confounder:

Socioeconomic status.

Why?

Socioeconomic status may affect feeding choice and infection risk through sanitation, crowding, maternal education, healthcare access, and nutrition.

So formula feeding may look harmful partly because the groups were different before feeding ever happened.

Confounding is one of the biggest weaknesses of observational studies because exposures are not randomly assigned. Observational studies can be rigorous, but lack of random assignment leaves them vulnerable to confounding and bias.

How it affects reasoning

Confounding is where bad researchers fool themselves.

They see an association and call it causation.

Wrong.

First principle:

An association is not a causal claim until alternative explanations are handled.

If you do not adjust for socioeconomic factors, you may blame the wrong thing.

5. Mediating variable

What it is

A mediator explains the pathway between exposure and outcome.

It answers: “How does the effect happen?”

Medical example

Question:

Does maternal education reduce under-5 mortality?

Here, care-seeking behavior, immunization, and nutrition may be mediators….!

Mediation asks whether the exposure affects the outcome through an intermediate mechanism. Epidemiology literature separates mediation from effect modification because mediation explains pathway, while effect modification asks whether the size or direction of effect differs across groups.

How it affects decision-making

Mediators help you design better interventions.

Example:

If maternal education reduces mortality mainly through earlier care-seeking, then the practical intervention is not just “educate mothers.” It is also transport, referral linkage, danger sign recognition, and reducing health system delay.

6. Moderating variable / effect modifier

What it is

A moderator or effect modifier changes the strength or direction of the relationship between exposure and outcome.

It answers:

“For whom does this work better, worse, or differently?”

Effect modification occurs when the effect of an exposure on an outcome depends on another variable.

Medical example

Question:

Does zinc reduce duration of diarrhea?

Possible moderator:

Baseline nutritional status.

How it affects decision-making

Moderators prevent lazy universal conclusions.

Bad conclusion:

“This intervention works.”

Better conclusion:

“This intervention works best in children under 2 years with moderate severity and early presentation, but not clearly in late referrals with hypoxia.”

That is the difference between research and clinical intelligence.

The mediator sits in the pathway.
The moderator stands outside the pathway and changes the effect.

7. Control variable

What it is

A control variable is a variable deliberately held constant, restricted, matched, stratified, or statistically adjusted.

It answers:

“What are we keeping stable so the comparison is fair?”

How it affects decision-making

Control variables protect fairness.

Example:

If one group has more preterm neonates and another has mostly term neonates, mortality comparison is useless unless gestational age is controlled.

Randomization is powerful because it aims to balance both known and unknown prognostic factors between groups, but reporting still matters. CONSORT was developed to improve RCT reporting so readers can appraise and interpret trial results properly.

8. Extraneous variable

What it is

An extraneous variable is unwanted noise that may affect the outcome but is not central to the research question.

It answers:

“What else could disturb the measurement or result?”

How it affects decision-making

Extraneous variables increase noise AND noise reduces your ability to detect truth.

Example:

If bilirubin samples are collected at different postnatal ages without standardization, the result may reflect timing rather than disease severity.

Extraneous variables do not always create systematic bias, but they reduce precision and reproducibility.

9. Categorical variable

What it is

A categorical variable places patients into groups.

It answers:

“Which group does this patient belong to?”

Types

Nominal = categories without order

Example: blood group, sex, organism type, mode of delivery

Ordinal = categories with order

Example: mild/moderate/severe dehydration

low/medium/high risk

grade 1/2/3/4 murmur

How it affects decision-making

Categorization is clinically useful but statistically costly.

Example:

Hemoglobin as a continuous variable gives more information. But hemoglobin grouped as mild/moderate/severe anemia is easier for treatment decisions.

The danger is crude grouping.

A child with Hb 10.9 g/dL and another with Hb 7.1 g/dL should not be treated as if they are clinically similar just because both are “anemic.”

10. Continuous variable

What it is

A continuous variable is measured on a numeric scale.

It answers:

“How much?”

How it affects decision-making

Continuous variables preserve information.

Example:

Oxygen saturation of 89%, 93%, and 97% are not the same.

But if you convert everything into “hypoxia: yes/no,” you lose clinical detail….!

Still, thresholds are necessary for decisions.

The threshold must be clinically justified, not invented after seeing the data.

11. Demographic variable

What it is

A demographic variable describes the patient or population.

It answers:

“Who are we studying?”

How it affects decision-making

Demographics decide whether evidence applies to your patients.

A pneumonia prediction rule developed in a high-resource urban hospital may fail in a rural Indian hospital where malnutrition, delayed referral, prior antibiotic use, and oxygen access are different.

This is called indirectness in evidence assessment. GRADE considers indirectness when judging certainty of evidence, because evidence from a different population, intervention, comparator, or outcome may not translate cleanly to the local decision.

13. How variables change healthcare decision-making

A. Clinical decision-making

Poor variable reasoning leads to wrong bedside decisions.

Example:

Study says:

“High CRP predicts antibiotic need.”

Problem:

CRP may simply reflect severity, duration of fever, referral delay, or previous antibiotics.

Better question:

“After adjusting for age, fever duration, respiratory rate, work of breathing, oxygen saturation, and danger signs, does CRP independently improve antibiotic decision-making?”

That is a useful clinical question.

B. Guideline-making

Guidelines should not ask only:

“Is the p-value significant?”

They should ask:

Is the exposure well-defined?
Is the outcome patient-important?
Were confounders handled?
Is there effect modification?
Is the result precise?
Is the population similar to ours?
Are harms measured?
Is it feasible?
Is it affordable?
Is it equitable?

C. Policy decision-making

A policy based on poor variable logic wastes money.

Example:

But the real drivers may be transport delay, poor referral, maternal anemia, prematurity, sepsis recognition, lack of SNCU beds, or poor postnatal follow-up.

Imagine an old family house. Kitchen, storage room, wiring, water pipes, a courtyard, rooms where people live. When the house is new, it tolerates abuse. Extra sacks of rice pushed into one corner. Old vessels in another. Broken furniture in the attic. Nobody notices much.

Then the years pass.

The storage room becomes full. But the family keeps bringing in more things. The room can no longer take it. Things spill into the kitchen. Then into the bedroom. Then near the electrical wiring. Then near the pipes.

At first, nothing dramatic happens. That is the dangerous part.

Why your grandfather’s butter/ghee is not the argument you think it is

Most adults today are not doing the physical work their grandparents did. Not walking long distances daily. Not lifting, bending, climbing, farming, cycling. But food supply has not reduced. In many homes it has increased…!

Breakfast is heavy. Tea has snacks. Lunch is polished rice or refined wheat in large portions. Evening has more tea and more snacks. Dinner is late. Sweets come because someone visited. Fried food comes because someone ordered. Packaged food comes because children asked. Festival food comes even when there is no festival.

The body receives fuel as if we are labourers.

But many of us live like office machines.

That mismatch is the beginning.

For some time, the body manages. It is genuinely intelligent. It stores excess energy in fat tissue. Fat tissue is not useless or evil — it is a living organ that stores energy, protects against sudden fuel overload, and talks to the brain and hormones. The body is trying to help you.

But every storage room has a limit.

Once safe storage becomes overloaded, the body starts pushing fat into places where fat does not belong. The liver becomes fatty — this is not a dramatic event, it happens silently over years. Muscle begins to resist insulin.

Here it is worth pausing. Insulin is the key that unlocks cells so sugar from your blood can enter and be used as fuel. When cells stop responding to this key — a condition called insulin resistance — sugar has nowhere to go. It piles up in the blood. The pancreas panics and makes more insulin to compensate. It works harder and harder. For years, the blood report may look almost normal, because the pancreas is still managing. Until one day it cannot.

Meanwhile, blood vessels are becoming inflamed. Triglycerides — the fat particles in blood — are rising. Blood pressure is climbing. The waist is growing. Tiredness is becoming the new normal.

Then one day the report comes.

But here is the part families are not ready for

This is no longer only an adult story.

We are watching the adult metabolic story arrive in childhood. And the tragedy is that most families are not seeing it, because they are looking for the wrong signs.

A child’s body is growing, learning, adapting, repairing. That gives childhood enormous resilience. But resilience is not immunity. A growing body can also be overwhelmed when the food environment is continuously working against it.

Today, many children are surrounded by a food system designed — and this word is chosen carefully, it is not an accident — designed to make them eat frequently, quickly, and without thought. Sweet drinks. Cream biscuits. Chips. Chocolate-flavoured “health” drinks. Fast food. Birthday food every week. Screen food. Reward food. Food after every small achievement. Food after every small sadness.

The child’s body receives the same signal again and again: Store. Store. Store.

And at the same time, many children move less than any previous generation. School is sitting. Tuition is sitting. Screen time is sitting. Transport replaces walking. Playgrounds disappear. Apartments replace open spaces. Parents fear traffic, injury, exams, heat, strangers, wasted time. So children are kept safe indoors — and slowly made metabolically unsafe.

Metabolism, in plain terms, is just the body’s system for converting food into energy and deciding what to do with the excess. When that system is working well, the child grows strong, sleeps deeply, plays without tiring, and feels hungry at the right times. When it is disrupted by continuous overload and under-movement, the same damage that takes decades in adults can begin arriving in a child within years.

A child with excess weight may already have a fatty liver. A teenager may already have insulin resistance. A school-going child may already have high triglycerides, early high blood pressure, or poor sleep quality. A young adolescent may show signs — a darkening of skin around the neck, irregular periods in girls, constant fatigue, snoring — that a doctor recognises as early metabolic warning signals.

These are not “adult diseases” arriving unexpectedly early.

They are the same disease, in the same body, starting earlier because the overload started earlier.

This should disturb us deeply.

A child is supposed to be building muscle, bone, confidence, sleep rhythm, and a healthy relationship with food. Instead, many children are being trained into craving cycles, sitting cycles, sugar cycles, screen cycles, and reward-eating cycles.

We are not only feeding children.

We are programming their metabolism.

Why families do not change even when they understand

Here is the part that most health writing skips.

Families are not stupid. They know fried food is not ideal. They know too much sugar is not good. They know children should move more. They have been told this hundreds of times.

And yet, the evening snack still appears. The sweet drink is still poured. The birthday celebration still involves cake plus chips plus sweet juice plus return gifts of more chocolate. The child still sits at the screen. The adults still sit at the screen too, right next to the child, asking why the child is sitting at the screen.

This is not weakness or ignorance. This is something more important: food in the Indian family is not just nutrition. It is love. It is welcome. It is apology. It is celebration. It is the answer to almost every emotion. A child cries — give a biscuit. A guest arrives — produce snacks immediately. Someone passes an exam — order sweets. Someone fails — order food to comfort.

Food has become our emotional vocabulary.

And changing that feels, to most families, not like a health decision but like an attack on how they express love.

This is the real storage room that is overflowing. Not just the pantry full of packaged snacks. But the family habit of reaching for food every time an emotion needs addressing. The belief — deep, unquestioned, passed across generations — that feeding is the most reliable form of care.

The person who says “eat less sugar” in that family is not heard as a health advisor. They are heard as someone who wants to remove warmth from the home.

This is why scolding does not work. This is why lectures do not work. This is why even fear — the diabetes diagnosis, the fatty liver report — works only briefly before the old patterns return. Because the food is not filling the stomach. The food is filling something else.

To change what the family eats, the family has to find other ways to say the things food was saying.

Sit with a child instead of giving a biscuit. Call instead of sending a box of sweets. Celebrate with an experience instead of always with sugar. Welcome guests with water and fruit without apology. Replace reward food with reward time, reward words, reward touch.

This is harder than buying low-calorie biscuits.

But it is the only change that lasts.

What honest families actually need to do

Not with drama. Not with crash diets. Not with body-shaming children. Not with fear. With ordinary discipline, applied quietly over ordinary days.

The house. Remove constant food visibility. If snacks are always on the table, people will eat them. That is not weakness — that is how human biology works. Keep fruit visible. Keep fried snacks out of daily reach. Do not convert the dining table into a shop.

Meal timing. The body does not need to be fed continuously from morning to night. Children need nourishment, not constant packaged snacks between meals. Adults do not need endless grazing. Tea does not need a snack every time. Night does not need a second dinner.

Muscle after meals. Ten to fifteen minutes of walking after lunch or dinner is not a small thing. It tells the body that fuel has arrived and the engines are running. Muscle is the body’s most powerful tool for clearing sugar from blood after a meal. An unused muscle is like a generator kept in the corner — large, expensive, switched off.

Play is not optional for children. Play is metabolic education. Running, jumping, climbing, cycling, swimming, dancing, rough-and-tumble games — these teach the body how to use fuel. They build muscle, sleep pressure, appetite regulation, coordination, and confidence. A child who never moves is not simply missing play. That child is missing a biological curriculum that no tuition class can replace.

Liquid sugar is the easiest fix. Sweet tea, soft drinks, packaged juices, and chocolate-flavoured health drinks quietly and quickly overload the system. Liquid calories enter the blood faster than solid food, leaving the body less time to respond. For families with children, reducing liquid sugar is one of the highest-impact changes with the least disruption.

Make the plate honest. Half the plate should not be polished rice or refined wheat at every meal. Add vegetables, dal, eggs, curd, pulses, fish, chicken, sprouts, or whatever fits the home, culture, and budget. Protein and fibre slow the flood of sugar into blood. They also keep hunger quieter for longer — which means less reaching for snacks by evening.

Sleep is not laziness. A tired brain is a brain that craves quick fuel — sweet, salty, fried. Poor sleep worsens hunger signals, insulin resistance, mood, and decision-making in both adults and children. Children who sleep late are not becoming modern. They are becoming metabolically fragile.

Do blood tests before fear arrives. Waist circumference, blood pressure, fasting sugar, HbA1c, lipid profile, and liver enzymes are maintenance checks, not disaster reports. For children, testing should be guided by the paediatrician — especially when there is significant weight gain, strong family history, or early warning signs. The goal is to catch the storage room overflowing before it reaches the wiring.

The mirror is not enough

Weight alone can mislead.

Some adults look thin but have fatty liver and high blood sugar. Some children look only “slightly healthy” but already have early metabolic risk. Some teenagers look normal but live on sugar, screens, poor sleep, and near-zero muscle use. Some people say, “I am not obese” — but their waist, their liver, their triglycerides, and their insulin response are telling a different story.

The weighing scale is not enough either.

The better question is not “How much do I weigh?”

The better question is: “Is my body still storing excess fuel safely — or has it started pushing that excess into organs where it does not belong?”

For children, the better question is not “Is my child thin or fat?”

The better question is: “Is my child becoming stronger, fitter, more active, better rested, and more regulated — or is the opposite happening?”

The body gives warnings long before collapse

The body is generous. It gives warnings long before it fails.

A growing waist is a warning. Fatty liver is a warning. Sleepiness after meals, rising triglycerides, borderline blood pressure, prediabetes — all warnings. Poor stamina in a child. A child who cannot play for twenty minutes without exhaustion. A teenager with constant fatigue, or irregular periods, or snoring, or a darkening of skin on the back of the neck — that last sign, which a doctor calls acanthosis nigricans, is the skin’s way of showing that insulin is working too hard.

These are not random symptoms. They are the house speaking….!

The difference between a family that hears these warnings and one that does not is rarely knowledge. Most families have the knowledge. The difference is whether the family has been honest — genuinely honest — about what food is doing in their home. Not just to the body. To the relationships. To the emotions. To the children who are watching every adult’s habits and learning what normal looks like.

Children do not build their food habits from lectures.

They absorb environments.

The home is the first and most powerful food environment a child will ever have.

Not everything. Not dramatically. Not all at once…!

The body is not a dustbin.

It is a home.

Homes do not collapse in one day. They collapse after years of ignored storage, ignored warnings, ignored signals, and the comfortable belief that the dampness in the corner is someone else’s problem to fix.

The storage room is full.

The question — the only question — is whether your family will start clearing it while the house is still standing.

There are weeks in clinical practice that stay with you long after the ward has moved on.

This was one of them.

Two families. Seven days. Both were from Shivamogga — not distant villages, not without resources, not without education. Both had arrived at our paediatric unit after long diagnostic journeys, with children who were getting worse and not better. The first family had a 14-year-old boy with a severe congenital neuromuscular disorder who had developed an overwhelming pneumonia. He was ventilated. His muscles were not recovering. The ventilator was doing work his body could no longer sustain. The second family had a five-month-old infant with persistent diarrhoea that had begun at two weeks of age and never resolved. We had done everything the setting allows — genetic panels sent to outside laboratories, biopsies, a full metabolic and infectious workup. No diagnosis emerged. The child had been admitted twice, perhaps three times. The illness continued its own silent course.

Both families had arrived at the same difficult threshold: treatment could still be continued, but it was increasingly unlikely to change the direction of what was happening.

After a few conversations, both families chose to take their child home.

Their children died there….!

Both families later told us that the final hours had been manageable. The grief was profound; it would be wrong to suggest otherwise. But what I noticed, in the weeks that followed, was that these parents did not come apart in the way I have sometimes seen. They had been prepared. And that distinction between; a family that was accompanied toward a death and one that was not, is what this piece is about.

Preparation does not prevent grief. It prevents bewilderment. Those are not the same thing, and the difference is entirely within our control.

Preparation Is Not What We Think It Is

Doctors often speak of families being in denial. Of waiting for them to accept reality. I have become cautious about both phrases.

A parent may understand every word we say about prognosis and still ask for one more treatment. They may speak of hope in the morning and funeral arrangements by evening. This is not irrationality. It is how people survive inside devastating uncertainty. Grief is not a corridor with a fixed endpoint, and the clinician is not a guide who must deliver everyone to the same destination on schedule.

There is a concept called anticipatory grief — the mourning that begins the moment a family understands a child’s life is threatened, long before the death itself. Clinicians sometimes treat it as a tool: something to facilitate in order to produce a smoother bereavement. The evidence is more cautious. Palliative interventions addressing anticipatory grief show benefit in some contexts, but families experience it in very different ways and the literature remains limited.¹ What anticipatory grief names is something families may already be living. Our task, maybe is to accompany it — not to manage it toward a target outcome.

What we can usefully do is more modest, and perhaps more important: tell the truth carefully, repeat it when necessary, ask what has actually been understood, make room for silence, attend to symptoms, and help parents think through the decisions that may soon confront them.

Preparation is not persuading families to agree with us. It is helping them see the road ahead clearly enough to make choices that remain their own.

These Conversations Are Usually Many Conversations

We sometimes imagine that breaking bad news is a single decisive event — the senior clinician sits down, the prognosis is explained, understanding follows, and a plan emerges. In practice, it is almost never like that.

With the first family, we spoke in stages. About the pneumonia. About the neuromuscular disease. About what the ventilator could and could not change. We also asked what they feared most and what they still hoped to protect. With the second family, uncertainty was the central fact — no diagnosis, no label, no clean trajectory. That made the conversations harder, not impossible. We could say what we knew: the child remained severely unwell, repeated admissions had not changed the course, and recovery was becoming increasingly remote. We could also say, plainly, what we did not know.

Prognostic humility is not evasiveness. Families deserve clarity. But clarity does not require false certainty — and a doctor who pretends otherwise is protecting themselves, not the family.

Structured frameworks such as SPIKES — developed originally in adult oncology and widely adapted since — help clinicians organise these conversations: an appropriate setting, understanding of what the family already knows, information delivered in manageable pieces, attention to emotion before strategy.² What no framework can substitute for is listening, or the kind of relationship that allows a parent to say what they are actually thinking. The discipline is to create that quality of attention regardless of how busy the ward is.

Research consistently shows that parents value clear information, honest communication, and preparation for what may happen near death.³·⁴ Good communication does not remove suffering. It ensures that avoidable confusion is not added to unavoidable grief.

If you leave a conversation feeling it has been completed, it probably has not. The next conversation is usually where real understanding begins.

What Preparation Actually Means

Preparation is sometimes reduced to a signed form, or a documented decision not to escalate treatment. It is much more than that.

A family considering care at home needs to know what changes in breathing to expect, how discomfort will be managed, whom to call at night, and whether returning to hospital remains possible if things become unmanageable. They need medicines, written instructions, and a number that will actually be answered. They also need something less clinical: permission. Permission to grieve before the death. Permission to disagree with each other. Permission to change their minds.

And they need one thing that is easy to overlook: the clear reassurance that choosing comfort-focused care is not the same as choosing the death. That a decision to stop escalation does not make them responsible for what is happening to their child.

Shared decision-making must not become a transfer of moral responsibility. Clinicians carry the weight of their recommendations alongside the family — not hand it over to them.

Home Is Not Always the Right Place

There is a quiet orthodoxy gathering around the idea of a good death at home, and I want to push back on it gently.

Some families prefer home. Others feel safer in hospital — close to nurses, oxygen, and immediate help. Preferences change as illness progresses. A child old enough to say something about it may feel differently from the adults around them.

The evidence is more cautious than the narrative. Systematic reviews of preferred place of death in children with life-limiting conditions show that many parents express a preference for home, but the literature is largely retrospective, concentrated in children with cancer, and drawn from settings with specialist palliative care — not from the majority of paediatric units in India where this decision falls entirely on the treating paediatrician.⁵ More recent integrative work finds that these decisions are shaped by the child’s condition, available services, the family’s capacity, and the needs of the household as a whole.⁶

The goal should not be to increase the number of children who die at home. It should be to increase the number of families who receive thoughtful care in a place they have been able to genuinely choose.

Sometimes that is home. Sometimes it is hospital. Neither is a moral achievement. Neither is a failure.

The Work That Remains After Treatment Changes

When cure is no longer possible, it is easy — I recognise this in myself — to feel there is little left to offer. Families sense this withdrawal even when no one says so aloud.

A change in the goal of treatment is not the end of care. It may require more careful medicine: relief of pain and breathlessness, management of secretions, attention to feeding, support for siblings, clear plans for likely crises. In most district hospitals across India, there is no specialist palliative care team to refer to. The paediatrician, the ward nurses, the community health worker — they carry the coordination. The absence of a formal service does not dissolve the need. It sharpens the responsibility.

Evidence suggests that parents’ support needs in these situations are varied and frequently unmet — particularly around communication, out-of-hours availability, and psychological care.⁷ We should not be surprised by this. We should be honest about it.

We should also be careful how we read families after a death. A composed parent at a follow-up appointment does not mean grief has resolved. Grief unfolds over months and years. A calm final day does not guarantee an uncomplicated bereavement.

What we can claim, and what is enough: the family was informed, accompanied, and given real choices. That is worth doing, even when we cannot measure exactly what it produced.

Preparation Without Promises

The two families from that week have stayed with me…!

Not because their choices offer a model. Not because I believe every family can be fully prepared for the death of a child, or that preparation guarantees anything about the grief that follows. They stayed with me because of something more specific — the visible difference between families who felt they had been told the truth gradually, honestly, with care, and families who had not. Both still grieve. But the quality of that grief appears different. Whether a controlled trial could capture that difference is a separate question. It is visible in the ward.

Parents do not need us to predict how they will mourn. They do not need to be hurried toward acceptance, or praised for being strong, or made to feel that remaining in hospital means they have somehow failed their child.

They need honesty that leaves room for uncertainty. They need information offered more than once. They need their questions taken seriously — including the ones that medicine cannot answer. And they need the reassurance that care continues even when cure cannot.

We cannot make such a loss orderly. We cannot promise that the grief will be gentler.

But we can ensure that when families reach the hardest part of this journey, they do not arrive there uninformed, unsupported, or alone.

How to use NotebookLM to think faster, research deeper, and write a stronger methodology chapter

Most researchers do not suffer from lack of information; but suffer from an excess of badly arranged information.

The average postgraduate student now has PDFs, guidelines, protocols, old theses, systematic reviews, departmental templates, and half-remembered advice from five different seniors. Everything is available. Nothing is organised. The student reads more and understands less. The faculty member collects more articles and writes with less confidence. The methodology chapter becomes a ritual paragraph factory, full of phrases like “a cross-sectional study will be conducted” and “data will be collected using a predesigned proforma,” as if methodology is a clerical formality.

It is not.

Methodology is the spine of research. If it is weak, the study may still collect data, but it will not produce knowledge.

That is where NotebookLM can become useful, not as a magic research machine, but as a disciplined research assistant. Google describes NotebookLM as an AI research and thinking partner that works with the sources you upload.

The first mistake is asking NotebookLM to “write my methodology”

That is the wrong prompt.

It is also the wrong mindset.

It must show why this design fits this question, why this population is appropriate, why this sampling method is defensible, why this tool measures what it claims to measure, why this analysis plan answers the objective, and why the limitations do not destroy the study.

Start with the Methodology Architect

Before choosing a design, upload your core material: the research problem, objectives, research questions, key literature, local setting, available resources, likely population, and any institutional constraints.

Then use the first prompt:

“Analyze the uploaded literature, research problem, objectives, research questions, and proposed study focus for [topic] and design a strong research methodology for my study. This is the correct beginning because it prevents premature method selection.

Many medical studies fail here. People choose cross-sectional design because it is easy. They choose convenience sampling because patients are available. They use a questionnaire because someone has a Google Form. Then they reverse-engineer the objective to fit the method.

That is not research design…!

NotebookLM should be used to expose that laziness early. Ask it to explain whether your proposed approach fits the research problem. Ask it what the design can and cannot answer. Ask it where the logic is weak. Ask it what assumptions you are making.

Sampling is not “whoever comes to OPD”

Sampling is where the action often begins….

Researchers say “random sampling” when they mean “whoever was available.” They say “consecutive sampling” without defining the recruitment period. They say “sample size was calculated” without explaining the assumptions. They include children, mothers, records, blood samples, or isolates without defining eligibility clearly. Then they expect the results to look credible.

They will not…

This matters especially in Indian medical college research. Our settings are messy. Patient flow is uneven. Records are incomplete. Follow-up is unpredictable. Referral bias is real. Tribal, rural, peri-urban, and tertiary-care populations are not interchangeable.

NotebookLM will not know these realities unless you upload or state them. So add a one-page “setting note” to every research notebook. Write plainly:

“This study will be conducted in a rural private medical college in Shivamogga. The hospital receives pediatric referrals from surrounding Malnad regions. Data collection needs choreography

A method section should not merely say “data will be collected.”

Who will collect it? When? From whom? Using which tool? At what time point? From records or direct interview? In which language? With consent? Who will verify missing data? How will biological samples be labelled, transported, stored, and processed? What happens if the patient is discharged early? What happens if the lab report is delayed?

AI does not know that the intern who collects the data may be posted in casualty next week. AI does not know that the OPD register uses a different diagnosis label from the ward case sheet. AI does not know that the freezer has space today but not during inspection season.

Measurement is where false confidence enters

The Instrument and Measurement Builder prompt is essential because researchers often confuse “having a tool” with “having a valid tool.”

A questionnaire is not valid because it looks neat. A checklist is not reliable because it has boxes. A scale is not appropriate because another paper used it. A translated tool is not automatically valid in Kannada because Google Translate produced readable words. A retrospective case sheet is not a reliable source just because it exists.

Ask NotebookLM to identify the best instruments, justify why they fit, and point out weaknesses.

For medical projects, this means asking hard questions:

Most students discover flaws too late. During departmental review. During IEC review. During thesis defence. During journal rejection.

This is especially useful because rigor differs by design.

A quantitative study needs clarity on internal validity, external validity, measurement reliability, confounding, precision, and statistical assumptions. A qualitative study needs credibility, reflexivity, dependability, transferability, audit trail, and saturation logic. A mixed-methods study needs integration, sequence, and explanation of how one strand informs the other.

Do not let NotebookLM use all these words as decoration.

Ask it to separate them:

“Which rigor criteria apply to my selected design, and which do not?”

This single question prevents a lot of academic nonsense and confusions.

A cross-sectional survey does not need confirmability in the same way a qualitative interview study does. A phenomenological study does not need Cronbach’s alpha unless it is also using a scale. A retrospective record review cannot pretend to have controlled exposure measurement if the records were not designed for research.

Remember, Methodology is not a vocabulary test…!

Data analysis should be planned before data collection

Many researchers collect data first and then go hunting for a statistician.

That is backward.

The phrase to use is:

“Create an objective-wise analysis plan.”

This should include:

• Objective.

• Variables.

• Data type.

• Descriptive statistics.

• Inferential test.

The correct workflow for a researcher

Here is the practical sequence.

Create one notebook for one project. Do not mix unrelated projects. Google’s help page states that each notebook is independent and NotebookLM cannot access information across multiple notebooks at the same time.

Upload only the core sources first. Start with 8 to 15 high-value documents: guidelines, systematic reviews, landmark studies, local data, previous protocol, departmental template, and your rough concept note. Do not dump 50 random PDFs on day one.

Add a “project context note.” This should include setting, population, resources, constraints, timelines, available lab support, likely feasibility issues, and what kind of output you need.

1. Ask for a source-wise summary. Check whether NotebookLM has understood the sources correctly.

2. Ask it to compare sources. Look for disagreements, not just common points.

3. Ask it to identify gaps. Then verify those gaps in PubMed or Scopus.

4. Use the methodology prompts in sequence. Do not jump to chapter writing.

The free version is powerful, but it is not unlimited and not infallible.

First, watch the source limit. Google’s current support page says a notebook can include up to 50 sources, and each source can contain up to 500,000 words or up to 200 MB for uploaded files. Do not waste that space on low-value documents. Upload fewer, better sources.

Second, remember that a source is a static copy. Google notes that an imported source is a static copy of the document you upload or import. If the original Google Doc, website, or guideline changes later, your notebook may not automatically reflect the updated version. For living guidelines and regulatory documents, recheck the source manually.

Third, check citations every time. NotebookLM provides citations, but citations are not a licence to stop thinking. Open the cited passage. Read the surrounding paragraph. Make sure the answer did not overinterpret a line.

Fourth, do not use it as a medical decision-maker. Google’s support page explicitly says not to rely on NotebookLM for medical, legal, financial, or other professional advice, and that such content is informational, not a substitute for qualified professional advice. For medical research, this means you can use it to organize literature and draft logic, but not to decide patient care, final diagnosis, treatment protocols, or ethical risk.

Fifth, do not upload material you have no right to use. Google’s source guidance tells users to avoid uploading documents they do not have rights to, and its support pages also warn users to respect copyright law. This matters for paid books, institutional files, unpublished manuscripts, confidential records, and student theses.

Sixth, be careful with patient data. De-identify everything. Remove names, phone numbers, hospital numbers, addresses, exact dates when not needed, and any combination of details that can identify a person. NotebookLM may be useful, but patient confidentiality is not negotiable.

Seventh, do not assume the mobile app has every feature. Google’s support pages repeatedly note that the NotebookLM mobile app may have limitations for some features, and some features such as Mind Maps are not supported on mobile at the time of the cited help page. Use the web version for serious research work.

Eighth, be aware of usage limits. Google states that limits apply to features such as Mind Map creation and that such limits are subject to change. Treat limits as a reason to work deliberately. Do not burn queries on vague prompts.

Ninth, avoid blind source dumping. If you upload poor literature, predatory papers, outdated guidelines, and irrelevant PDFs, NotebookLM will faithfully help you build a poor argument.

Tenth, remember that NotebookLM is not a database search engine. It works on what you give it. It can help you understand uploaded sources. It cannot guarantee that you have found all relevant evidence. PubMed, Cochrane, Scopus, Embase, clinical trial registries, ICMR guidance, NMC documents, and local policy sources still matter.

7. Qualitative vs quantitative vs mixed methods: which should I choose?

The design depends on the question. Not on fashion…!

Choose quantitative if your question asks “how many” or “how much”

Use quantitative cross-sectional design when you want to measure:

• Prevalence.

• Frequency.

• Risk factors.

• Association.

• Scores.

• Clinical parameters.

• Laboratory values.

• Knowledge scores.

Example:

“What proportion of MBBS students have adequate knowledge of antimicrobial stewardship?”

Best method: quantitative.

Tools: structured questionnaire, lab values, clinical records, validated scales.

Choose qualitative if your question asks “why” or “how”

Use qualitative methods when you want to understand:

• Beliefs.

• Barriers.

• Experiences.

• Reasons.

• Perceptions.

• Decision-making.

• Cultural context.

Example:

“Why do mothers delay seeking care for neonatal danger signs?”

Best method: qualitative.

Tools: interviews, focus group discussions, thematic analysis.

Do not force this into a questionnaire too early. You will get shallow answers.

Choose mixed methods when numbers alone are not enough

Use mixed methods when you need both measurement and explanation.

Example:

“Prevalence of vaccine hesitancy and reasons for vaccine refusal among parents.”

Quantitative part:

How many parents are hesitant?

Which factors are associated?

Qualitative part:

Why are they hesitant?

What fears, stories, family influences, and social media factors drive refusal?

Mixed methods is powerful but harder. Do not choose it unless you have time, skill, and supervision.

8. Common wrong choices

1. If the study is only prevalence, do not call it mixed methods.

2. If you are only using open-ended questions in a Google Form, that does not automatically make it qualitative.

3. If you are only adding two quotes in discussion, that is not qualitative research.

4. If you are doing interviews but converting everything into percentages, you have probably misunderstood qualitative research.

5. If you want to study lived experience, do not use only multiple-choice questions.

6. If you want to estimate prevalence, do not rely on five interviews.

The method must match the question.

9. What outcomes are suitable in cross-sectional studies?

Good outcomes:

• Disease present or absent.

• Nutritional status.

• Anemia status.

• Blood pressure category.

• Knowledge score.

• Vaccination status.

• Antibiotic use.

• Screen time category.

• Sleep quality score.

• Depression score.

• Quality of life score.

• Adherence status.

• Awareness level.

• Microbiological isolate.

• Resistance pattern.

Bad outcomes for cross-sectional studies:

• Recovery.

• Relapse.

• Mortality after treatment.

• Progression.

• Survival.

• Treatment failure over time.

• Development of disease in future.

Those need cohort, case-control, trial, or follow-up design.

10. Can I use hospital records?

Yes.

A cross-sectional study can be:

• Community-based.

• Hospital-based.

• School-based.

• Record-based.

• Laboratory-based.

• OPD-based.

• Ward-based.

But you must be honest about the denominator.

Example:

If your study is from a tertiary hospital, do not claim community prevalence.

You can say:

“Prevalence among children attending the pediatric OPD.”

Not:

“Prevalence among children in Shivamogga.”

Hospital studies are useful, but they are biased toward people who seek care.

11. Can I study risk factors?

Yes, but carefully.

You can study factors associated with disease.

You cannot prove they are risk factors unless temporality is clear.

Example:

Low birth weight and developmental delay in a cross-sectional study.

You can say:

“Low birth weight was associated with developmental delay.”

But even here, because birth weight clearly occurred before current development, the argument is stronger.

Example:

Depression and poor sleep among medical students.

You cannot say poor sleep caused depression. Depression may cause poor sleep. Both may be linked to stress. Cross-sectional design cannot separate this.

12. What is temporality, and why does it matter?

Temporality means knowing which came first.

Cause must come before effect.

Cross-sectional studies usually measure exposure and outcome together. So temporality is weak.

Example:

Obesity and low physical activity.

Did low physical activity cause obesity?

Or did obesity reduce physical activity?

Or did both result from diet, environment, and family habits?

A cross-sectional study cannot answer this fully.

That is why causal claims are dangerous….!

13. Sample size: what do people get wrong?

People calculate sample size mechanically without understanding the objective.

For prevalence studies, you need:

• Expected prevalence.

• Precision.

• Confidence level.

Formula commonly used:

n = Z²pq / d²

Where:

p = expected prevalence.

q = 100 minus p.

d = absolute precision.

For association studies, sample size should ideally be based on expected difference between groups or expected odds ratio.

Common mistake:

Using prevalence formula for an analytical study with multiple associated factors.

This may be accepted for UG projects, but it is not ideal.

Another mistake:

Using 50 or 100 because it is convenient.

Convenience is not sample size justification.

14. Sampling: what is best?

The best sampling method depends on setting….!

For community studies:

• Simple random sampling.

• Systematic random sampling.

• Cluster sampling.

• Stratified sampling.

For school studies:

• Cluster sampling by school or class.

For hospital OPD studies:

• Consecutive sampling is often practical.

For lab/record-based studies:

• All eligible records during a defined period.

Bad sampling:

“Whoever responded to the WhatsApp form.”

This creates selection bias….!

Google Form studies are often weak because the sample is self-selected, literate, online, and motivated.

15. What is the minimum acceptable methodology?

A proper cross-sectional study must clearly state:

• Study design.

• Study setting.

• Study duration.

• Study population.

• Inclusion criteria.

• Exclusion criteria.

• Sample size calculation.

• Sampling method.

• Study variables.

• Operational definitions.

• Data collection tool.

• Validation of questionnaire, if applicable.

• Data collection procedure.

• Statistical analysis plan.

• Ethical considerations.

• Consent process.

• Bias and limitations.

If any of these are missing, the protocol is incomplete….!

16. What variables should I collect?

Do not collect everything. Collect what answers the question.

Usually:

• Sociodemographic variables.

• Clinical variables.

• Exposure variables.

• Outcome variables.

• Possible confounders.

Example: anemia study in adolescent girls.

Collect:

• Age.

• Class.

• Dietary pattern.

• Menstrual history.

• Worm infestation history.

• Socioeconomic status.

• BMI.

• Hemoglobin.

• Iron supplementation history.

Do not collect irrelevant details like father’s mobile type unless you have a reason.

Every variable must have a purpose….!

17. What are confounders?

A confounder is a third factor that distorts the relationship between exposure and outcome.

Example:

Screen time and obesity.

• Socioeconomic status may affect both screen time and obesity.

• Diet may affect obesity.

• Physical activity may affect obesity.

If you ignore these, your association may be misleading.

Cross-sectional studies must identify obvious confounders in advance.

For analysis, use stratification or regression where appropriate.

18. What statistical tests are commonly used?

For descriptive data:

• Mean and standard deviation.

• Median and interquartile range.

• Frequency and percentage.

• Prevalence with 95% confidence interval.

For association:

• Chi-square test.

• Fisher’s exact test.

• Independent t-test.

• Mann-Whitney U test.

• ANOVA.

• Correlation.

• Logistic regression.

• Linear regression.

But statistics should not drive the study. The research question should…!

19. What is better: prevalence ratio or odds ratio?

For cross-sectional studies, prevalence ratio is often more intuitive.

But many studies use odds ratio because logistic regression is common.

The issue: when the outcome is common, odds ratio can exaggerate the association.

Example:

If 40% of students have poor sleep, odds ratio may look stronger than the real prevalence ratio.

For UG/PG studies, odds ratio is acceptable if interpreted carefully.

Do not call it “risk” unless the design supports risk estimation.

20. What makes a questionnaire-based study weak?

Weak questionnaire studies usually have these problems:

• No clear research question.

• No validated tool.

• Too many vague questions.

• Leading questions.

• Poor translation.

• No pilot testing.

• No scoring system.

• No plan for missing data.

• No definition of “good knowledge” or “positive attitude.”

• No justification for cutoff scores.

A questionnaire is not a study. It is only a tool.

21. Can I create my own questionnaire?

Yes, but then you must do basic validation.

At minimum:

• Literature review.

• Expert review.

• Content validation.

• Translation and back-translation if using local language.

• Pilot testing.

• Reliability testing, usually Cronbach’s alpha for scales.

• Clear scoring system.

For UG projects, a simple expert-validated questionnaire may be acceptable.

For publishable work, stronger validation is needed.

22. What is the difference between KAP and real behavior?

KAP studies measure what people say.

Not always what people do.

A mother may know exclusive breastfeeding is important but may not practice it due to family pressure, work, pain, poor support, or misinformation.

A doctor may know antibiotic stewardship but still prescribe broad-spectrum antibiotics because of patient pressure, diagnostic uncertainty, or hospital culture.

So KAP studies are useful, but they are shallow unless interpreted with context…!

23. What biases are common in cross-sectional studies?

Selection bias:

The people included are not representative.

Recall bias:

Participants do not remember past exposures accurately.

Social desirability bias:

Participants give answers that sound acceptable.

Non-response bias:

Those who refuse may differ from those who participate.

Survivor bias:

Only those alive or available are studied.

Measurement bias:

Poor tools or inconsistent measurement.

Confounding:

A third variable distorts the association.

A good protocol admits these risks before the reviewer points them out…!

24. What are good cross-sectional study topics for medical students?

Strong UG topics: (Only easy examples to understand)

• Prevalence of anemia among adolescent girls.

• Screen time and sleep quality among adolescents.

• KAP regarding antibiotic use among caregivers.

• Prevalence of smartphone addiction among medical students.

• Nutritional status among under-five children attending immunization clinic.

• Vaccination coverage among children aged 12 to 23 months.

• Prevalence of depression/anxiety among medical students using validated scales.

• Hand hygiene compliance among healthcare workers.

• Clinical profile of febrile seizures in children.

• Antimicrobial susceptibility pattern of urinary isolates in children.

• Vitamin D status among children with recurrent respiratory symptoms.

• Awareness of cervical cancer vaccination among mothers.

These are feasible.

25. What topics look attractive but are usually flawed?

• “Impact of AI on medical education.”

Too broad.

Better:

“Awareness and self-reported use of AI tools among undergraduate medical students.”

• “Effect of social media on mental health.”

Too causal.

Better:

“Association between social media use and depression scores among adolescents.”

• “Risk factors for sepsis mortality.”

Needs follow-up or cohort.

Better:

Needs longitudinal design.

Better:

26. What should the objectives look like?

Good objective:

“To estimate the prevalence of anemia among adolescent girls in selected schools of Shivamogga.”

“To assess factors associated with anemia among the study participants.”

Bad objective:

“To study everything about anemia.”

A good cross-sectional study usually has:

One primary objective.

Two to three secondary objectives.

Do not overload it.

27. What should the title look like?

Good title format:

“Prevalence of [condition] and associated factors among [population] in [setting]: a cross-sectional study.”

Examples:

“Prevalence of anemia and associated factors among adolescent girls in selected schools of Shivamogga: a cross-sectional study.”

“Knowledge, attitude, and practices regarding antibiotic use among caregivers of children attending a tertiary hospital: a cross-sectional study.”

“Clinical and microbiological profile of urinary tract infection among children admitted to a tertiary care hospital: a cross-sectional study.”

28. What should not appear in the title?

Avoid:

• Impact.

• Effect.

• Outcome.

• Prediction.

• Risk.

• Causation.

Unless the design truly supports it.

Use:

• Prevalence.

• Pattern.

• Profile.

• Association.

• Awareness.

• Practice.

• Burden.

Distribution.

29. How do I decide between hospital-based and community-based?

Choose hospital-based if:

• You need clinical cases.

• You need lab confirmation.

• You need patient records.

• You need disease profile.

• You have limited time.

Choose community-based if:

• You need true population burden.

• You need prevalence in the general population.

• You are studying health behavior, awareness, nutrition, screening, immunization, or environmental exposure.

Hospital-based studies are easier.

Cross-sectional does not mean harmless…!

You still need ethics approval if human participants, patient data, biological samples, identifiable information, or sensitive questions are involved.

Consent is needed for direct participant data collection.

Waiver of consent may be possible for anonymized retrospective record review, depending on IEC decision.

Sensitive topics need privacy…

Examples:

• Mental health.

• Sexual behavior.

• Substance use.

• HIV status.

• Domestic violence.

• Academic performance.

• Financial status.

31. What is a good analysis plan?

A clean analysis plan says:

1. Data will be entered in Excel/SPSS/R.

2. Categorical variables will be summarized as frequency and percentage.

3. Continuous variables will be assessed for normality.

4. Mean with SD or median with IQR will be used.

5. Prevalence will be reported with 95% confidence interval.

6. Associations will be tested using chi-square/Fisher’s exact test for categorical variables.

7. Appropriate tests will be used for continuous variables.

8. Multivariable logistic regression will be performed for variables significant in univariate analysis or clinically relevant variables.

9. A p-value less than 0.05 will be considered statistically significant.

But do not blindly write regression if your sample size is small.

32. What makes the discussion section strong?

A good discussion does not repeat results.

It explains:

• What was found.

• How it compares with previous studies.

• Why it may be different.

• What it means clinically or socially.

• What the limitations are.

• What future studies should do.

Bad discussion:

“Our study is similar to other studies.”

Good discussion:

“The higher prevalence in our study may reflect the tertiary-care setting, where children with more severe illness are overrepresented.”

That is interpretation….

33. What are the standard limitations?

Common limitations:

• Cross-sectional design prevents causal inference.

• Single-centre study limits generalizability.

• Hospital-based sampling may introduce selection bias.

• Self-reported data may cause recall and social desirability bias.

• Small sample size may limit subgroup analysis.

• Unmeasured confounders may affect associations.

Do not hide limitations. State them clearly.

A mature limitation section increases credibility.

34. What makes a cross-sectional study publishable?

Publishable cross-sectional studies usually have:

• Relevant question.

• Clear population.

• Adequate sample size.

• Valid measurement tools.

• Representative sampling.

• Strong operational definitions.

• Appropriate statistics.

• Confidence intervals.

• Honest limitations.

• Clinical or public health relevance.

• Clean writing.

A boring topic with good methods is better than a fashionable topic with weak methods…!

35. What is the simplest decision rule?

Use this:

• If you want to know “how common”, choose cross-sectional.

• If you want to know “what is associated”, cross-sectional may work.

• If you want to know “what caused it”, cross-sectional is weak.

• If you want to know “what happens later”, choose cohort.

• If you want to compare treatment effect, choose trial.

• If you want to understand lived experience, choose qualitative.

• If you want both numbers and reasons, choose mixed methods.

Final practical checklist

Before approving any cross-sectional proposal, ask:

1. What exactly is being measured?

2. Who is being studied?

3. Where are they being studied?

4. When are they being studied?

5. Is this a prevalence/profile/association question?

6. Is the title free of causal language?

7. Is the sample size justified?

8. Is the sampling method realistic?

9. Are variables clearly defined?

10. Is the tool validated or at least expert-reviewed?

11. Are confounders considered?

12. Is the analysis plan appropriate?

13. Are limitations acknowledged?

14. Is the conclusion likely to stay within the limits of the design?

If the answer is no, the study is not ready….!

One-line rule for students

A cross-sectional study can tell you what exists and what is associated with it, but it usually cannot tell you what caused it or what will happen next…

Not an animal ethics committee. A human ethics committee — an Institutional Ethics Committee for research on human participants. I know what that means. I know what the forms look like, what the committee quorum must be, how the minutes should be structured, what a harm-benefit analysis is supposed to contain, when to return a protocol and what language to use when returning it. I have spent years on the inside of this machinery. I know its logic, its vocabulary, its patience, and its limits.

So when I recently found myself needing to conduct an animal experiment — a small one, by any measure — I assumed the experience of navigating an animal ethics process would be broadly familiar. One committee. One application. One set of principles.

I was wrong in ways I am still processing….!

Not wrong about the paperwork. The paperwork is abundant, demanding, and detailed, and I understood that. What I did not expect was the thought that arrived somewhere between reading the guidelines and drafting the application — the thought that wouldn’t leave, that kept rearranging itself into new and uncomfortable shapes.

The thought is this: we have built a society in which a researcher who wants to use five mice to understand a disease mechanism must justify every aspect of that use to a committee of scrutineers who will ask hard questions, demand revisions, and require documentation across multiple forms — while a person who wants to kill the same species for sport, for dinner, for festival, or for the pleasure of a photograph encounters, in practice, no comparable obligation to explain themselves at all.

That is not a paradox in the loose, literary sense of something that seems contradictory but isn’t. That is a genuine structural asymmetry, built into law, embedded in governance, and sustained by political convenience. And working inside an ethics committee for years had, somehow, made it harder for me to see — not easier.

The moment the machinery became visible

There is a form. It is called Form B.

It asks you — in the careful, unhurried language of institutional bureaucracy — to justify the life you are about to use. Species. Number of animals. Objective of experiment. Justification for animal use. Alternative methods considered. Expected pain and distress level. Humane endpoint criteria. Anaesthesia and analgesia protocol. Disposal method. Veterinary certification. Signatures etc….

Sitting with that form as an applicant — rather than as the person on the other side who reviews it — produces a very specific kind of clarity. Every question on that form is a good question. Every question reflects a genuine moral concern. Every question is asking: have you thought carefully about whether this is necessary, whether it can be done more gently, whether the animal’s suffering is being taken seriously?

Reading those questions, I thought: yes. This is what ethics looks like when it is functioning.

And then I went home, drove past a livestock truck on the highway — the kind that anyone who has driven on an Indian road has seen, crowded and open-sided and fast — and thought: no committee reviewed what is happening in that truck. No form was filed. No humane endpoint was defined. No alternative was considered. No one was asked whether the number could be reduced…..!

The truck moved through the world with the frictionless confidence of a thing that has never been required to justify itself.

That is when the thought arrived and refused to leave.

I am not the first person to notice this. I am probably not saying anything that informed people in animal welfare, bioethics, or food systems haven’t said more rigorously and at greater length. What I am offering is something more specific: the view from inside an ethics committee of the wrong type — the experience of a person who knows intimately how this machinery works, who believed that knowledge was transferable, and who discovered instead that the machinery for animal research ethics and the machinery for the rest of our relationship with animals are not two versions of the same thing. They are two entirely different projects, built in different centuries, from different anxieties, toward different ends.

What the law actually built — and what it didn’t

Let me be precise from my side, because some degree of precision is what this kind of argument requires.

In India, animal experimentation is governed under the Prevention of Cruelty to Animals Act, 1960, and the rules framed under it for the breeding of and experiments on animals. The Committee for the Purpose of Control and Supervision of Experiments on Animals — CPCSEA, now restructured as the Committee for the Control and Supervision of Experiments on Animals, CCSEA — oversees institutional animal ethics committees across research establishments. The Breeding of and Experiments on Animals (Control and Supervision) Rules, 1998, amended in 2001 and 2006, are not guidelines. They are enforceable requirements. Establishments must register. Experiments must receive prior committee approval. Records must be maintained. Inspections can and do happen. Violations carry consequences.

The moral core of this system is stated plainly: “animals should not be subjected to unnecessary pain or suffering before, during, or after experiments.” Every word in that sentence is doing work. Unnecessary means the experiment must be affirmatively justified. Pain means physiological suffering counts. Suffering means psychological distress counts. The premise of the whole framework is that causing harm to an animal requires a reason, and the reason must be examined by people who are not simply the person who wants to cause the harm.

That is not a trivial ethical achievement. That is a sophisticated institutional structure that took decades of activism, scientific reflection, and legislative effort to build. A researcher cannot simply decide that twelve mice are necessary. The researcher must argue for twelve mice, and someone else — a committee of people with competing perspectives, including an animal welfare representative and a socially aware citizen, not just scientists — must evaluate the argument.

Now look at what the law built for food.

The Food Safety and Standards Act, 2006, governs slaughterhouse regulation alongside state-level rules, municipal licensing, and provisions under the Prevention of Cruelty to Animals Act that nominally cover transport and slaughter. The rules exist. Slaughterhouses are required to be licensed. Transport of animals is supposed to comply with standards. There are provisions on the books against unnecessary cruelty during transit and at the point of killing.

But nowhere in the food system is there a body that asks, before the slaughter: is this necessary? Nowhere is a harm-benefit analysis mandatory. Nowhere is a farmer or a hotel or a caterer asked to consider whether the number of animals used can be reduced. Nowhere is the question of alternatives formally posed. The food system has hygiene inspectors, food safety officers, and licensing authorities. It does not have anything that functions like what we have built for the laboratory….!

The word unnecessary appears in both legal domains. It carries operational weight in one. In the other, it is present on paper and absent in practice — a word without administrative electricity….!

That is not an oversight, but a choice. A choice made so long ago, and so thoroughly absorbed into the default assumptions of governance, that it no longer presents itself as a choice. It presents itself as the natural order of things….

The three exemptions nobody voted on : Food, hunting, and research are different moral problems

Somewhere in the evolution of law and custom — in India and across most of the world — society quietly granted three exemptions from the kind of moral scrutiny we have built for the laboratory. There was no referendum. No committee was convened. The exemptions accumulated through agriculture, religion, commerce, and habit, until they became indistinguishable from common sense.

The first exemption is appetite. When an animal is killed so that a human being can eat, the killing inherits a presumption of acceptability that is so deeply embedded it is almost invisible as a presumption. Food is personal, cultural, is livelihood and is the domain where nearly everyone, regardless of their views on other matters, feels entitled to be left alone. The state, which is ultimately a collection of people who also eat, has been consistently reluctant to interfere with what goes onto the plate — even when what goes onto the plate involves substantial animal suffering upstream, in transport, in holding conditions, in slaughter. The food system has been granted, in effect, a cultural immunity from the kind of question we ask routinely in the animal house: is this suffering necessary?

The second exemption is ritual. When an animal is killed for ceremony — religious festival, cultural practice, communal observance — the killing acquires a different protection. It is wrapped in the language of faith, tradition, constitutional rights, and community identity. These are not trivial considerations. I am not dismissing them. I am noting that the ritual framing has, historically, functioned as a moral silencer. We do not require that festival slaughter submit a harm-benefit analysis. We do not ask whether the number of animals can be reduced, whether alternatives exist, whether the endpoint is humane. The ceremony is the justification.

The third exemption is wealth, and it operates most nakedly in trophy hunting. In India, the Wild Life (Protection) Act, 1972, under Section 9, prohibits hunting. The prohibition is real. But globally — and in ongoing conversations about wildlife management — trophy hunting occupies a morally incoherent middle space. In various jurisdictions it functions under conservation justifications, permit systems, quota management, and expensive vocabulary about sustainable use. The practical reality is that sufficient money can, in the right geography, transform the killing of a rare and extraordinary animal into an invoice, a photograph, and a conservation claim.

None of these three exemptions are identical. Trophy hunting and factory farming are not the same problem. Ritual slaughter and sport killing are not morally equivalent. I am not collapsing them into a single category.

I am pointing to the structural feature they share: none of them require the kind of prior, documented, committee-reviewed, alternatives-considered justification that we require of a scientist who wants to use five mice to study diabetes.

The scientist must argue for the mice.

The diner, the devotee, and the devoted trophy hunter do not argue at all.

What it actually takes to even begin an animal experiment : Ethics committees protect animals, but also institutions

Since I have recently been on the applicant’s side of this process, let me describe it accurately.

To conduct animal experiments in a registered institution in India, the institution must first be registered with CCSEA. The animal facility must meet species-specific environmental standards — defined temperature ranges, humidity levels, light-dark cycles, bedding specifications, cage sizes, socialisation requirements. A qualified veterinarian must be attached to the facility. Animal care personnel must be trained. Daily health monitoring records must be maintained. The institution must constitute an Institutional Animal Ethics Committee with a mandated composition: a biological scientist, a veterinarian, a scientist from a different institution, a nominee from a recognised animal welfare organisation, and a socially aware citizen — not only scientists, not only insiders.

This committee must meet at defined intervals. Its minutes must be recorded and maintained. All approvals must be documented. Adverse events must be reported. The whole system is subject to inspection and registration renewal.

And then, when a researcher submits a protocol to this committee, it is evaluated against the 3Rs — the framework of Replacement, Reduction, and Refinement first articulated by Russell and Burch in their 1959 work, The Principles of Humane Experimental Technique, now globally embedded in research ethics. Can the animal be replaced by a validated non-animal model? Can the number of animals be reduced without compromising statistical validity? Can the procedure be refined to minimise pain, distress, and suffering?

Only after this process — which can take weeks or months, and frequently involves revisions, clarifications, and resubmissions — does the experiment receive approval to proceed.

I am not describing this to complain about it. The process produces better science and more defensible ethics. Researchers who navigate rigorous IAEC review tend to produce more carefully justified protocols, more considered endpoints, more thoughtful procedures. The 3Rs are not bureaucratic performance. I am describing it because I want you to hold that image — the registration, the committee composition, the veterinary oversight, the form, the revision, the approval — and then look at the livestock truck moving at speed on the highway outside. No welfare pre-assessment. No replacement considered. No reduction required. No refinement mandated. No committee. No form. No minutes. No record.

The uncomfortable reason this happened — and why hypocrisy is the wrong word

The tempting explanation for all of this is hypocrisy. Society cares about mice/rats in laboratories but not chickens/sheep/pigs/cattles in trucks. That is hypocritical. Move on.

I feel that explanation is satisfying yet wrong. Calling it hypocrisy implies that people know the principle and choose not to apply it. The more accurate explanation is structural, and in some ways more disturbing.

Animal research is tightly regulated because it is institutionally legible. An animal house has a physical address. A protocol has a number. A researcher has a name and an employer. A committee has a chairperson and a register. A cage has a health monitoring record. The state regulates what it can see, and what the state can see is what has an address, a record, and a responsible named individual attached to it.

The food system is not evading scrutiny because it is conspiring to hide. It is ungovernable at the moral level because it is too large, too economically embedded, too culturally diffuse, and too electorally dangerous to interrogate at every point. The chain of responsibility runs from the farmer to the transporter to the trader to the slaughterman to the processor to the retailer to the restaurant to the caterer to the consumer to the app to the delivery worker to the festival to the economy. When responsibility is sliced that thin, no single point can bear the full moral weight. Each point can point to the next. The consumer can point to the system. The system can point to demand. Demand can point to culture. Culture can point to history. History can point to necessity. Necessity becomes invisible……!

In the laboratory, responsibility is concentrated and named.

In the food chain, responsibility is distributed until it disappears.

I feel, this is why the laboratory gets ethics and the market gets logistics. It is why the inspector visits the animal house and not the banquet. It is why Form B exists and the menu does not have an equivalent.

Governments regulate what they can see. They ritualise what they cannot control. They moralise what is institutionally neat. They normalise what is economically dangerous to disturb.

The result is an ethics system that is rigorous exactly where rigour is administratively convenient, and absent exactly where it would cost something real.

The hunter’s argument, and why I find it uncomfortable rather than simply wrong

Trophy hunting deserves its own examination, because the trophy hunter makes an argument that is, in a specific and limited sense, more honest than most of what surrounds this debate.

The trophy hunter says: I am willing to travel far and pay substantial sums for the experience of killing a significant animal. I am not pretending it is for survival. I am not claiming nutritional necessity. I am not sheltering behind cultural tradition. I am saying directly that I want the experience — the pursuit, the danger, and the kill — and I am willing to pay for it. In some formulations of this argument, the permit fee funds conservation, local employment, and anti-poaching infrastructure, making the death a net ecological benefit.

I find this argument, at its core, repellent. I also find it structurally more honest than much of what the food industry says about itself…..!

The food system routinely claims necessity where the evidence for necessity is thin. For the majority of people in urbanised economies, meat consumption is a preference sustained by price, habit, culture, and taste — not a biological imperative. The trophy hunter does not claim necessity. The diner often does, implicitly, by framing dietary choice as if it were beyond the reach of ethical scrutiny.

I am not arguing that the hunter is right. I am arguing that if the hunter is wrong — which, in the case of trophy killing of rare species, I believe — the basis for that wrongness cannot rest only on the motive of desire rather than necessity. If it did, that argument would need to be applied far more broadly than most people in this debate are comfortable applying it.

The trophy hunter holds up a mirror. The reflection is uncomfortable precisely because it is accurate about more than hunting.

What I am actually asking for : The animal does not know our file system

In my honest opinion, essays of this kind usually arrive at this section and request something that cannot be implemented, then retreat into irony.

I will try to be more specific.

I am, in no way, asking for the end of meat-eating. That argument is not mine to make, and it is not where this observation leads. I happen to be vegetarian — a personal fact that I mention (at the end) here and do not intend to labour, because the argument I am making is independent of dietary identity and available equally to anyone who eats meat and also works in research or governance. The paradox does not require a vegetarian to notice it. It requires only that someone sit with Form B for long enough to look up.

I am not asking for the end of animal research. Some animal research is definitely necessary. The 3Rs framework, when taken seriously, represents genuine moral progress in how science relates to its non-human subjects. Regulatory oversight of institutional animal use is not a problem to be solved. It is an achievement to be maintained.

What I am asking for is this: an end to the pretence that what we have constitutes a coherent animal ethics system.

It is not a system. It is a patchwork — historically accumulated, separately constructed, never integrated, never subjected to the question that would expose its incoherence: if this principle matters here, why does it not matter there? The 3Rs are not applied to the food chain. The harm-benefit analysis is not required of the festival. The humane endpoint is not defined for the trophy wall. These are not oversights. They are political decisions wearing the clothes of common sense.

The minimum I am asking for is the willingness to ask the question — formally, institutionally, without cultural immunity — in the places where it has never been asked. Can some of this consumption be replaced? Can the number of animals be reduced? Can the conditions of transport and slaughter be refined toward something closer to what we demand of the laboratory?

Not banning. Not lecturing. Just asking.

Because the questions we ask of a scientist before approving five mice for a metabolic study are not complicated questions. They are basic questions. The only reason they feel radical when applied elsewhere is that we have decided, without ever deciding, that certain domains are beyond the reach of the kind of scrutiny we take for granted in others…

The conclusion I did not expect to reach : the address decides the ethics

So no, I am not confused anymore.

I began this process trying to navigate an animal ethics application. I ended it with a clearer view of a system I had been working inside for years without fully seeing.

The moral infrastructure we have built around laboratory animals is, by almost any measure, more rigorous than what we have built around any other category of animal use. The IAEC process — the registration, the committee, the 3Rs, the veterinary oversight, the documentation — asks the right questions. It asks whether harm is necessary. It asks whether alternatives were considered. It asks whether suffering was minimised. It asks who is responsible. It demands answers in writing and retains those answers in a record that can be inspected.

The rest of our animal use does not ask these questions. Not systematically. Not institutionally. Not with signatures and minutes and a nominated animal welfare representative in the room.

And the asymmetry, I have come to think, is not accidental. It is revealing. We built rigorous ethics around the domain — scientific research — where institutional trust is claimed, public funding is at stake, and reputational risk is concentrated and nameable. We built permissive ethics, or no ethics, around the domain — food, festival, sport — where economy, culture, and political arithmetic make scrutiny expensive.

The mouse in the laboratory has a committee because the institution that houses it has accepted a public obligation, and the public has decided to hold it to that obligation.

The animals in the supply chain do not have a committee because we, as a society, have collectively decided — not in any single law, not in any single moment, but in the accumulated weight of everything we have left unlegislated — that appetite does not owe an explanation to anyone…!

I sit on an ethics committee. I now understand, more precisely than I did before, that there is no unified animal ethics in this country or most others. There is one ethics for the address and another for the supply chain. One ethics for the protocol and another for the plate. One ethics for the institution that claims public benefit and another for the economy that claims cultural exemption.

I have filled out the form.

I remain unsure whether the form, and the system it represents, is a genuine moral achievement — or whether it is the one room in which society performs the seriousness it has decided not to extend anywhere else.

Both things may be true at once.

That is the most uncomfortable conclusion available, which is usually a reliable sign that it is correct.

1. The “epidemic” is mostly a counting change

CDC data show about 1 in 31 US children now diagnosed, up from 1 in 150 in 2000, but the hosts argue most evidence does not support a true biological surge. It is portrayed as an epidemic, but that framing is misleading.

2. Autism has two core features, described since 1943

Leo Kanner first described autism as a “disorder of affective contact” — from the Greek for “alone.” Today the definition still rests on: (a) difficulties with social communication, and (b) repetitive behavior or “insistence on sameness.”

3. The diagnostic boundaries have been repeatedly widened

Over 40 years, criteria loosened: PDD-NOS and Asperger’s syndrome (1990s) captured people close to but not meeting classic autism. In 2013, DSM-5 collapsed them into “autism spectrum disorder,” allowed diagnosis by history, and made criteria work across developmental ages.

4. Better tools and less stigma explain much of the rise

We now have structured interviews and play-based assessments that did not exist 30-40 years ago. Parents are also less devastated and more informed — they often suspect autism before the clinician does — so more cases are identified and counted.

5. Adult diagnoses are surging, especially among women

A JAMA Network Open study found diagnoses in 26- to 34-year-olds rose 450% between 2011 and 2022. Historically, criteria and training videos were built around boys, so girls were missed. Mild traits remain male-skewed, but severe autism with intellectual disability is much more gender-balanced.

6. Genetics is the primary risk factor; environment matters modestly

Twin and family studies since the late 1970s point to genetics. About 15% of autistic people carry a rare, identifiable gene change, more often in profound autism. Other replicated risks: extreme preterm birth, short interval between pregnancies, maternal infections, and advanced parental age.

7. Vaccines and acetaminophen are not causes — rubella vaccine prevents autism

After controlling for fever and familial factors, acetaminophen shows no effect. The vaccine link is “heartbreaking” to keep studying because congenital rubella is one of the clearest preventable causes of autism, so prenatal rubella immunity testing and vaccination are protective.

8. Outcomes hinge on co-occurring conditions, not autism severity alone

IQ and language ability predict independence far better than the number of autistic traits. “Profound autism” is now used informally to mean autism plus intellectual disability and minimal functional communication, often with higher rates of irritability or self-injury when needs cannot be expressed.

9. Getting diagnosed in the US is a “diagnostic odyssey”

Pediatricians screen, but families must simultaneously pursue a specialist evaluation and early-intervention services, usually while on long waitlists. No US system automatically coordinates this. Autism Speaks’ “100-day kit” exists because parents must become navigators, and insurance often will not pay for a navigator — paradoxically making help more available to Medicaid than private-insurance families in some states.

10. Treatment is early, play-based support — not a cure, and the field is shifting

The strongest evidence is for early intensive behavioral intervention starting soon after diagnosis through about first grade. Modern approaches happen through play, aiming to build communication and flexibility, not to make a child appear neurotypical. It can raise measured IQ by ∼15 points in studies but does not erase autism. Clinicians are hopeful about earlier diagnosis, growing autistic self-advocacy, gene-targeted therapies for profound autism, and better prevention of depression and suicide, which are more common in autistic people.

Critics, including the Endocrine Society, have raised alarms about the framework’s requirement that obesity directly cause a comorbidity to qualify as “clinical.” The podcast highlights type 2 diabetes as a flashpoint: if the disease must be proven to stem directly from adiposity, some patients with diabetes could be excluded from a clinical obesity diagnosis, despite diabetes being one of obesity’s most prevalent complications. The Lancet Framework: From Anthropometry to Organ Dysfunction

The Commission’s central innovation is a two-tier diagnostic architecture that moves beyond BMI as a standalone measure. Under the new framework, excess adiposity must first be confirmed by either direct body-fat measurement (e.g., dual-energy X-ray absorptiometry or bioelectrical impedance) or at least one additional anthropometric criterion—waist circumference, waist-to-hip ratio, or waist-to-height ratio—alongside BMI, with thresholds tailored to age, sex, and ethnicity. For individuals with BMI greater than 40 kg/m², excess adiposity is presumed without further confirmation.

Once adiposity is established, the framework stratifies patients into two categories:

• Preclinical obesity: Excess adiposity without demonstrable organ dysfunction or functional limitation. This state is analogized to prediabetes—a period of heightened risk that warrants monitoring, lifestyle counseling, and preventive intervention, but not necessarily pharmacologic or surgical therapy.

• Clinical obesity: A chronic, systemic illness characterized by functional alterations in tissues or organs attributable to excess adiposity. Diagnosis requires evidence of dysfunction across 18 prespecified criteria spanning 11 body systems—including cardiovascular, metabolic, hepatic, renal, respiratory, musculoskeletal, and reproductive systems—or substantial, age-adjusted limitations in activities of daily living.

The Commission explicitly recommends that BMI be relegated to a population-level screening tool rather than an individual diagnostic measure. The framework has been endorsed by at least 76 professional organizations worldwide, including the American Diabetes Association, the American Heart Association, and the American Association for Metabolic and Bariatric Surgery.

Empirical Validation: Evidence from the All of Us Cohort

The first large-scale empirical test of the Lancet framework emerged from the National Institutes of Health’s All of Us cohort, published in JAMA Network Open in late 2025. Among 301,026 adults, the new definition increased obesity prevalence from 42.9% (BMI-only) to 68.6%—a 60% rise driven largely by the inclusion of “anthropometric-only obesity,” individuals with normal BMI but elevated waist-based measures who would previously have been classified as healthy.

Critically, the framework demonstrated meaningful risk stratification. Preclinical obesity, while less severe, was not benign: it conferred significantly elevated risks of incident diabetes (AHR, 3.32) and cardiovascular events (AHR, 1.40), though no significant increase in mortality. Among adults aged 70 years or older, 78% met criteria for obesity under the new definition, and 54% had clinical obesity.

These findings support two key propositions: first, that anthropometric-only obesity captures a clinically relevant phenotype previously invisible to BMI-based systems; and second, that the preclinical/clinical distinction effectively stratifies short- and long-term risk, albeit with the caveat that preclinical obesity remains a high-risk state warranting active management rather than passive observation.

Critical Appraisal: The Endocrine Society’s Dissent

Despite the framework’s conceptual elegance, it has encountered substantial resistance. In April 2026, the Endocrine Society published a guideline communication in The Journal of Clinical Endocrinology & Metabolism articulating three principal objections that strike at the operational core of the Commission’s proposal.

Causation and Diagnostic Burden. The requirement that clinicians demonstrate organ dysfunction is caused by excess adiposity introduces an evidentiary standard that is difficult to satisfy in routine practice. Distinguishing obesity-attributable heart failure from heart failure with coincident obesity, for instance, demands clinical judgment that may vary by setting, specialty, and resource availability. Critics warn that this could delay treatment, create documentation barriers, and restrict access to pharmacotherapy and metabolic-bariatric surgery.

The Instability of “Preclinical.” The Endocrine Society argues that preclinical obesity is conceptually fragile: its detection depends on the intensity of diagnostic testing. A patient subjected to comprehensive metabolic, cardiovascular, and sleep evaluations may cross into clinical obesity, while the same patient in a resource-limited setting would remain preclinical. This testing-dependent variability risks arbitrary classification and inequitable care.

The Diabetes Exclusion. Perhaps the most contentious element is the Commission’s decision not to include type 2 diabetes as an automatic criterion for clinical obesity. Because the framework requires that organ dysfunction be directly caused by adiposity, and because diabetes is classified as an independent disease entity, some patients with type 2 diabetes and obesity may not qualify for a clinical obesity diagnosis. The Endocrine Society deems this inconsistent with decades of evidence establishing obesity as the primary driver of type 2 diabetes pathophysiology.

Conceptual Debates: Diagnosis vs. Staging

The Lancet Commission’s lead author, Francesco Rubino, has countered that critics are conflating two fundamentally different clinical operations: diagnosis (determining whether disease is present) and staging or risk stratification (determining what to do about it). In this view, the Commission’s framework functions like a pregnancy test—it establishes a binary state of illness—whereas tools like the Edmonton Obesity Staging System (EOSS) or the King’s Obesity Staging System (KOSS) function like antenatal care pathways, guiding management intensity.

Rubino further argues that incorporating diabetes into the diagnostic criteria for clinical obesity would represent a category error: diseases are defined as distinct entities, and one chronic disease does not become a diagnostic criterion for another. This aligns with the Commission’s broader philosophical aim—to establish obesity as an independent disease with its own pathophysiological signature, rather than a mere risk factor that requires the presence of “comorbidities” to justify treatment.

Yet this distinction, while logically coherent, may not fully address the pragmatic concerns raised by clinicians. The EOSS, for example, has demonstrated robust predictive validity for mortality and cardiovascular events by integrating metabolic, functional, and mental health domains into a severity score. The CDC has suggested that integrating the Lancet Commission’s diagnostic criteria with established staging systems like EOSS could offer a more granular approach, distinguishing, for instance, between clinical obesity with heart failure (high cardiovascular risk) and clinical obesity with urinary incontinence (lower cardiovascular risk but significant functional impairment).

Clinical and Policy Implications: Access, Equity, and Cost

How obesity is defined is not an academic exercise; it is a determinant of who receives treatment and who pays for it. Under the traditional BMI-based model, insurance coverage for anti-obesity medications and metabolic-bariatric surgery often requires the presence of specific comorbidities—effectively treating obesity as a risk factor rather than a disease. The Commission explicitly argues that clinical obesity, as a distinct chronic illness, should not require the presence of another disease to justify coverage.

However, the 60% increase in obesity prevalence observed in the All of Us cohort under the new definition carries profound resource implications. If nearly seven in ten adults—and more than three-quarters of those over 70—meet criteria for obesity, health systems must grapple with the fiscal and infrastructural demands of expanded screening, anthropometric measurement, and longitudinal monitoring. The framework’s call for waist circumference protocols, body composition scans, and organ-system evaluations requires time, training, and equipment that are unevenly distributed across clinical settings, particularly in low-resource and rural environments.

The Endocrine Society’s warning that the framework could “exacerbate existing health inequities” is therefore not speculative. If preclinical obesity elicits a “watchful waiting” approach while clinical obesity demands intensive intervention, the boundary between these categories becomes a rationing mechanism. Patients in well-resourced systems, with access to comprehensive diagnostics, may more readily qualify for clinical obesity and thus for pharmacologic or surgical therapy. Patients in under-resourced settings, where organ dysfunction may be underdetected, risk being relegated to the preclinical category with delayed or denied access to effective treatments.

Pediatric and Population Considerations

The Commission’s framework also faces unresolved challenges in pediatric populations. While BMI has well-established age- and sex-specific percentiles for children, standardized thresholds for waist circumference and waist-to-height ratio in youth remain underdeveloped across ethnic groups. Moreover, BMI alone has been shown to be a highly specific measure of fatness in children, raising questions about the incremental value of additional anthropometrics in this demographic. The lack of biomarkers distinguishing visceral from subcutaneous fat further limits precision in both adults and children.

At the population level, the Commission’s recommendation to reserve BMI for epidemiological surveillance while using more refined measures for individual diagnosis creates a dual-track system. This is scientifically sensible but administratively complex: public health surveillance, clinical trials, and insurance claims databases have historically relied on BMI for interoperability. Transitioning to a multi-metric system will require harmonization of measurement protocols, cutoff points, and data infrastructure—a task that demands international coordination and sustained funding.

This scenario is hauntingly familiar. Recently , Dr. Nikhil Agrawal shared a near-identical experience from his pediatrics internship on X. A child with prolonged fever and diarrhea tested positive for HIV. The accompanying “parents” were tested: mother positive, the man presented as father negative. Shaken, he returned alone with urgent, probing questions: “Does HIV spread from kissing?” No, casual contact does not transmit the virus. Then, the second question: “What about sex?” Yes—sexual contact is a primary route. The truth emerged: the man was the child’s uncle (chacha). The mother had been living with him, while the biological father lived with the uncle’s wife (chachi). Both biological parents had known their status but chose not to disclose it to their new partners. One pediatric case had quietly become a contact-tracing web spanning four adults and two households.

I encountered a strikingly similar situation during my own residency. The details differed slightly, but the emotional and ethical architecture was identical. That single diagnosis dismantled the comforting illusion that medicine deals only in closed biological systems.

It revealed instead a dense network of human relationships—love, betrayal, stigma, silence—where a virus moves along invisible fault lines of trust. What followed was a masterclass in the messy intersection of cutting-edge science, cultural realities, legal safeguards, and timeless ethical tensions.

The Science That Changed Everything: Treatment as Prevention (TasP) and U=U

Decades ago, an HIV diagnosis carried a different weight—fear, inevitability, isolation. Today, the landscape has transformed through Treatment as Prevention (TasP), one of public health’s most powerful innovations. The principle is simple: when people living with HIV (PLHIV) take antiretroviral therapy (ART) consistently and achieve sustained viral suppression, they cannot sexually transmit the virus.

This is crystallized in the U=U campaign—Undetectable = Untransmittable. Landmark studies provide strong evidence. The HPTN 052 trial demonstrated that early ART reduced linked transmissions by 93–96%, with no linked infections once viral suppression was stable. PARTNER, PARTNER 2, and Opposites Attract studies tracked tens of thousands of condomless sex acts in both heterosexual and MSM serodiscordant couples: zero genetically linked transmissions from partners with undetectable viral loads.

The World Health Organization (WHO) affirms that people with an undetectable viral load (typically <20–50 copies/mL) do not transmit HIV sexually, and risk is negligible even at suppressed levels ≤1000 copies/mL. This knowledge reframes everything in cases like the one on the ward. Disclosure conversations shift from pure fear to empowerment. A suppressed viral load plus U=U education can reduce stigma and encourage testing. The Ethical Tightrope: Balancing Four Principles in Real Time

This is where medicine transcends biology. The basic four pillars of medical ethics—autonomy, beneficence, non-maleficence, and justice—become living tensions.

Autonomy grants individuals control over their bodies and information. Yet the biological parents’ choice to withhold their status curtailed the new partners’ autonomy—the right to informed decisions about their health and relationships. Autonomy carries shadow responsibilities.

Confidentiality builds trust essential for testing and care. Without it, people avoid care. But it is not absolute. When there is clear, foreseeable risk to identifiable others, the duty to protect shifts the balance.

Truth-telling emerges as both simple and profound. Accurate information about transmission routes, U=U, and modern outcomes must be “delivered with empathy” spacious enough to hold betrayal, fear, and hope simultaneously.

India’s Legal Framework: Safeguarded Partner Notification

India’s HIV and AIDS (Prevention and Control) Act, 2017 provides a rights-based scaffold. Section 9 carefully regulates disclosure to partners:

• Only a physician or counselor (not any healthcare worker) may disclose.

• Strict conditions apply: significant risk to the partner, counseling of the index patient who is still unlikely to disclose, prior notification of intent to the index patient, and in-person counseling of the partner.

• Crucial safeguard: Disclosure is prohibited if it risks violence, abandonment, or severe harm—especially relevant for women.

The Act imposes a general duty on PLHIV (post-counseling) to take reasonable precautions against transmission, which can include disclosure or risk reduction (condoms, ART leading to U=U). You look at the certificate. It says AI Expert. Or AI Practitioner. Or sometimes, with admirable ambition, AI Leader. The organisation that issued it is three months old. The trainer has fifteen thousand followers and a course that costs three thousand rupees. The session was four hours on a Sunday. There was a PDF. There was a Canva poster. There were testimonials. There was a QR code. And now there is a certificate, laminated or otherwise, making its slow journey down the feed between the words “grateful” and “honoured.”

You scroll past. But something stays. A low-grade unease. Not quite anger. Not quite contempt. Something closer to grief — because you know what learning actually costs, and it does not cost a Sunday afternoon….!

Why this market is so easy to build

India has always been vulnerable to the certificate.

We like laminated proof. We like abbreviations after names. We like the language of transformation — upskilling, reskilling, future-readiness, industry-alignment — because it promises that a single investment of time and money can lift us across the gap between what we are and what we fear we must become. This is not a character flaw. It is the reasonable anxiety of a population where competition is fierce, stakes are real, and the distance between aspiration and opportunity is often crushing.

A contemplation

Here is what makes the fake AI course economy so easy to construct, and so difficult to expose. Teaching someone to use AI — to prompt it, navigate it, extract from it, display its outputs — is no more intellectually demanding than teaching someone to use a calculator. A calculator is a profound technological achievement. It performs arithmetic faster and more accurately than any human mind. But we do not issue certificates in calculator proficiency. We do not hold masterclasses on pressing the right buttons. We do not call someone mathematically transformed because they can retrieve a square root without a pencil. We understand, intuitively, that the calculator is a tool — and that the intelligence required to use it well lives not inside the device, but inside the person who knows what question to ask, what the answer means, and what to do with it next.

AI is a vastly more sophisticated instrument, but the principle is identical. “The cleverness”, as Penrose would say, lives in the machine. The understanding must still live in the human. What the fake course industry has discovered — with remarkable efficiency — is that operating the tool can be demonstrated in four hours, monetised with a poster, and certified with a PDF.

What it cannot teach, because it does not itself possess, is the judgment to know when not to trust the tool, what the tool is actually doing beneath its confident surface, where it fails, and what it means for the specific discipline you inhabit. That understanding requires something slow, conscious, and domain-grounded. You cannot WhatsApp it to someone. You cannot Canva it into a certificate. And you certainly cannot manufacture it on a Sunday afternoon…!

The fake mentor is not merely incompetent. He is useful.

There is a version of this story where the fake trainer is simply a charlatan and the student is simply a victim. That version is comfortable but incomplete.

The fake mentor is not just bad at his job. He is performing a different job entirely. His job is not to create capable learners. His job is to create satisfied customers. A satisfied customer has received what they came for — the certificate, the photograph, the public proof, the feeling that they have done something decisive about their fear (FOMO). Whether they can actually use AI for anything consequential is irrelevant to this transaction.

The fake mentor is recognisable if you know what to look for. He does not say “I don’t know.” He does not say “this is more complex than I can cover today.” He does not say “you need foundational understanding before these shortcuts make any sense.” He speaks in slogans because slogans are faster than slow truth. He simplifies what should not be simplified, and he does so with extraordinary confidence, because confidence is exactly what the anxious learner is buying. A mentor who reveals complexity looks weak. A mentor who sells certainty looks masterful. The market rewards performance…!

There is a particular form of fraud that nobody calls fraud because everyone benefits in the short term. The trainer profits. The learner gets a certificate. The institution gets footfall. The algorithm gets engagement. The only loser is the future version of the learner — the one who sits down to solve a real problem, with real consequences, and discovers that the certificate has nothing to teach him.

For learners in medicine and healthcare, this is not merely disappointing. It is dangerous. Teaching doctors to use AI without discussing many nuances about hallucination/confabulation, clinical/academic accountability, ethics/evidence hierarchy, and researcher/patient safety is not education/certification. It is negligence dressed as innovation. When the AI confidently recommends the wrong information, the certificate will be nowhere in the room…!

The fake learner is not innocent

Here is the part that stings. It is more comfortable to be the aggrieved student than to examine what we actually wanted from the course.

Many learners — not all, but many — do not want the discipline of learning. They want the appearance of learning. They want the certificate, the update, the signal to peers and parents and potential employers that they are not standing still. They do not want to struggle with first principles. They do not want to sit with confusion long enough for understanding to arrive. They want to be declared upgraded.

Real learning is not like this. Real learning is slow. It makes you feel inadequate before it makes you capable. It is uncomfortable in a specific way — the discomfort of realising how much you do not know, which is the only honest starting point for knowing anything. It requires revision. It requires being corrected. It requires producing something that can be judged — not a certificate, but actual work: a diagnosis, a written argument, a decision with consequences. Something that can fail…!

The fake trainer and the fake learner complete each other quietly. The trainer performs expertise. The learner performs seriousness. The certificate is the sacred object exchanged between them. Both leave satisfied. Nobody asks whether learning occurred. Nobody wants to ask, because asking would disturb the arrangement.

This is the uncomfortable mirror. Behind every fraudulent AI masterclass is not just a fraudulent trainer. There is also a learner who preferred the shortcut, and a participant who confused payment with investment. The system holds because all three need it to hold…!

What a real mentor actually does

The simplest test of a mentor is this: does the student become more independent over time?

A real mentor creates adults. Not fans. Not followers. Not recurring customers for the next certificate programme. Adults — people who can think, question, filter, produce, and judge without needing the mentor in the room. This means a real mentor does things that are commercially uncomfortable. They say “I don’t know.” They design tasks the student might fail at, because failure is where learning lives. They give feedback that is honest rather than affirming. And they make themselves, slowly and deliberately, unnecessary.

The best mentors do not build audiences. They build the capacity in others to outgrow them. That is a profoundly different ambition from the WhatsApp masterclass economy, where every answer is also a door to the next paid module, and where the learner’s growing dependency is mistaken — by both parties — for the mentor’s growing value.

Real AI mentorship would be domain-specific and pedagogically grounded. It would include genuine assessment — not examinations for their own sake, but evidence of ability; rooted in Penrose’s very distinction between cleverness and intelligence, is the one no four-hour Sunday certification will ever teach. Because to answer it, you would need to understand the domain deeply enough to know where the machine’s confidence ends and genuine knowledge begins.

The question that remains

We are not the first generation to sell symbols instead of substance. India has always had its certificate culture, its observerships and CME credits, its appreciation trophies for entering a room. What is different now is the scale, the speed, and the weight of the moment. A genuine technological transformation is underway. The gap between those who truly understand it and those who merely perform understanding of it will have real consequences — for patients, for students, for institutions, for the country’s ability to use this moment rather than merely Instagram it.

The problem is not that AI courses exist. AI should be taught — to doctors, teachers, engineers, students, researchers, schoolchildren — calibrated carefully to their context, need, and stage. A medical student needs AI literacy that includes hallucination, clinical accountability, data privacy, and evidence appraisal. A school student needs something built around curiosity, verification, and disciplined writing. A researcher needs something else entirely. One poster, one Sunday, one PDF fits none of them.

The problem is that we have allowed the word learning to be replaced by the word certification. We have allowed a market to tell young people that proof is more important than ability. We have allowed the anxious parent, the impatient student, and the opportunistic trainer to conspire — not always maliciously, but structurally, habitually, mutually — toward the same hollow outcome.

There is a question worth sitting with. Not which certificate to collect next, but the older, slower, more inconvenient one.

A real mentor creates independence. A fake mentor creates a market. And a fake learner keeps that market alive…!

That sentence indicts everyone. The trainer who exploits anxiety. The student who buys shortcuts. The parent who mistakes price for quality. The institution that mistakes events for education. The society that mistakes cleverness — artificial or otherwise — for intelligence….!

Penrose’s word for what these machines possess is cleverness. It is a good word. It is honest. Let us apply it equally to the ecosystem that has grown around them. A cleverly designed poster. A cleverly worded promise. A cleverly timed payment link. A cleverly laminated certificate. And at the centre of it all, a learner who was clever enough to attend — but never given the chance to understand…!

When real work arrives and real judgment is tested, the laminated illusion will quietly fail. And what will remain is what should have mattered from the beginning.

It has no syllabus. No hall ticket. No OMR sheet. No NTA oversight — which, given recent events, some might consider an advantage…!

It runs twenty-four hours a day, seven days a week, across WhatsApp groups, Telegram channels, X threads, Instagram reels, Reddit posts, and YouTube comment sections. The candidates are familiar. NEET aspirants. MBBS students. Interns. Residents. Post-PG drifters. Repeaters. First-generation doctors. Burnt-out seniors who should know better but keep scrolling anyway.

The questions are not from Physics, Chemistry, or Biology.

They are worse.

“Which is the best branch?” “Is medicine even worth it?” “Is DNB equal to MD?” “Does merit actually matter?” “Is private practice dying?” “Is burnout normal?” “Will life settle after residency?” “Is USMLE an escape plan or just another expensive trap?”

And the answers come wrapped in a vocabulary so confidently deployed and so magnificently undefined that it deserves its own chapter in the history of Indian medical education…..

Everyone Knows It. Nobody Can Perform It.

Here is a phrase you have heard in the last week: “That department has a good academic atmosphere.”

Now close your eyes and (try to) define it.

Go ahead. Take your time…..

You cannot. Neither can I. Neither can the person who said it. But we all nodded, because that is how the handshake works. You perform it. You receive it. You move on. No definitions exchanged, no clarity achieved, full social transaction completed.

This is the operating language of Indian medical Gen Z — a dialect built from tens and hundreds of phrases that sound intelligent, feel meaningful, circulate endlessly, and dissolve into fog the moment you ask what they actually mean.

Good clinical skills. Lifestyle branch with money. Toxic department. Tier-1 college. Skill-based branch. Research culture. Burnout is normal. Sacrifice phase. You will understand later. Connections matter. Merit matters. Luck matters more……!

Every single one of these is discussed daily — on social media, in hostel corridors, in mess halls, in post-call WhatsApp groups at 3 a.m. — by a generation of medical students trying to make sense of a system that has never spoken to them plainly….

And right now, after the cancellation of NEET UG 2026; officially pulled by the NTA following the examination conducted on 3 May, referred to the CBI, re-examination dates pending — that language is louder, more anxious, more desperate, and more urgent than ever before…!

We Built the Vagueness First

Before we diagnose Gen Z’s language, let us do something uncomfortable.

Let us look in the mirror.

Who first said “good clinical skills” without defining what good meant, which skills were being assessed, or how they would be evaluated?

We did.

Who told students to become “good doctors” without explaining whether that means diagnostic accuracy, communication, ethics, procedural competence, documentation, teamwork, humility, or all of the above in what proportion?

We did.

Who described a college as “academically strong” without showing a single outcome metric, research output figure, or faculty development record?

We did.

Who said “this student has good bedside manner” without a rubric, a framework, or even a second opinion?

We did.

Medical education has always had a vocabulary of vagueness. We invented it, we taught it, we rewarded it, and we used it to avoid the hard work of defining what we actually expect and how we actually assess it.

Gen Z “did not” (entirely) corrupt our language. They inherited our corruption. Then they put it on the internet….

And the internet did what the internet always does — stripped the phrases of context, injected them with anxiety and sarcasm, distributed them to millions of people, and served them back to us as content….

We created the fog. They are just living in it and making reels about it.

The Night NEET Broke Something Bigger Than an Exam

For a policy administrator, a cancelled examination is a logistical problem.

For a student, it is a psychological event.

Let us not confuse the two.

The student who spent two years turning down weddings, friendships, sleep, and sometimes sanity; reducing an entire human life to a rank predictor score, does not experience the cancellation as an administrative inconvenience. The first-generation aspirant whose parents borrowed money, whose relatives whispered doubts, whose entire family’s social mobility is pinned to a medical seat; that student does not simply “prepare again” when told the examination was cancelled and is under CBI inquiry.

They open their phones.

They go to the only place that has ever spoken to them at 2 a.m. without judgment.

And there, the language mutates fast.

“Merit is dead.” “System is rigged.” “Drop year wasted.” “Escape India.” “Medicine is not worth it.”

This is not overreaction. This is what happens when institutional trust; already thin, already cracked by years of paper leaks, quota controversies, rank manipulation allegations, and counselling chaos — fractures publicly one more time….!

A MedEd facilitator who cannot read this moment will misread the student.

They will see laziness where there is exhaustion. They will see cynicism where there is grief. They will see entitlement where there is a legitimate question about whether the system ever intended to be fair. They will see a meme where there is an early warning sign.

The Great Branch Circus

No discussion of Gen Z medical language is complete without a proper salute to the best branch debate — the greatest unresolved argument in Indian medical social media, running continuously since approximately 2009, showing no signs of resolution, generating content at industrial scale.

Every branch has been described, in the last six months alone, as simultaneously: the best branch, saturated, AI-proof, going to be killed by AI, underrated, overrated, lifestyle, toxic, money-making, underpaid, academically rich, intellectually dead, family-friendly, and not for anyone who wants a life.

Dermatology: lifestyle with money - until someone says saturated. Radiology: AI will kill it - until someone says interventional is AI-proof. Surgery: skill-based, respect, legacy - until someone says toxic culture, no sleep, no life. Psychiatry: future branch - until someone says who will pay. Pathology: safe lifestyle - until someone says boring and corporate-dependent. Community Medicine: only if you want academics - said with the energy of a warning, not a recommendation.

Here is the truth beneath the circus:

A 23-year-old choosing a specialty in India is making a decision worth forty years of their life - with poor data, family pressure, rank anxiety, coaching mythology, financial stress, peer comparison, social-media distortion, gender constraints, and almost no structured mentorship.

Then we wonder why they ask strangers on Telegram.

If the system will not provide career maps, students will crowdsource them.

They will get fifty contradictory answers.

They will come back the next day and ask again.

This will continue until someone builds real career counselling infrastructure — not motivational speeches, not alumni talks, not “follow your passion” energy, but actual branch-wise data on training quality, income variability, workload reality, private practice barriers, global pathways, AI disruption, gender considerations, and capital requirements.

A mature student can handle truth. What confuses students is not truth, but the fog.

“Toxic” Is Doing Too Much Work

The word toxic has become a suitcase in which an entire generation is trying to pack a wardrobe.

It now contains: a senior who shouted once. A department that systematically humiliates. A consultant who teaches nothing. A unit that runs on resident sleep deprivation and calls it training. A ward where errors are punished but never analysed. A college where thesis work is unpaid clerical labour. A culture where feedback is structurally unsafe. A hospital where students are bodies, not learners.

When one word means everything, administrators can pretend it means nothing.

“These kids call everything toxic.”

Which is exactly why MedEd facilitators must do the work of unpacking this language rather than dismissing it.

When a student says toxic department, the correct response may not be: “This generation is too sensitive.”

The correct response maybe a set of specific questions.

Is there humiliation? Is there unsafe duty duration? Is there academic neglect? Is there caste, gender, regional, or language bias in how students are treated? Is there sexual harassment? Is there unpaid labour? Is there no supervision, no feedback, no grievance mechanism? Is there a documented fear of retaliation?

These are governance questions, not sensitivity questions.

Satire is useful until diagnosis begins. After that, we need governance.

The Mythology of Suffering

“Emergency duty builds character.” “Residency is meant to be hard.” “Burnout is normal.” “Sacrifice phase.” “After PG life settles.” “You will understand later.” “Everyone struggles.”

Read these in sequence and you have the complete philosophical infrastructure of a system that has converted structural failure into personal virtue…!

There is a difference between difficulty and abuse. Difficulty teaches. Abuse deforms. Difficulty has purpose, proportion, supervision, recovery, and feedback. Abuse has chaos, fear, exhaustion, silence, and a senior who will later say “I also went through this.”

Medical training must be demanding. Nobody with clinical sense argues otherwise. But toughness is not produced by cruelty. Judgment is not produced by sleep deprivation. Clinical courage is not produced by humiliation. Professional maturity is not produced by silence enforced until the trainee becomes senior enough to enforce it on the next person.

“Burnout is normal” is the most dangerous sentence in contemporary medical culture.

Common is not normal. Normal is not acceptable. Acceptable is not inevitable…!

And inevitable is often just the word used by people who have no intention of changing anything…!

Gen Z has decoded this faster than many of us are comfortable admitting. That is why their sarcasm irritates senior doctors. Not because it is always correct, but because it exposes the poverty of our old explanations. It is uncomfortable to be mocked accurately.

Noble Profession. Sure. But Noble For Whom?

“Noble profession” is invoked most reliably in three situations.

When recruiting students into medicine. When hospitals want unpaid extra work done without complaint. When someone asks for better pay, reasonable hours, or protection from violence.

A profession cannot keep demanding nobility from those inside it while delivering them uncertainty, debt, exploitation, hierarchy, moral injury, and a cancelled entrance examination under CBI investigation.

Gen Z uses noble profession with irony now. This is not cynicism. This is intelligence. They are not rejecting ethics. They are rejecting emotional blackmail dressed as ethics. They know the difference. The educator’s task is not to restore blind reverence to a system that has not earned it. It is to restore moral seriousness to a profession that deserves it.

Medicine needs ethics. It does not need slogans.

What We Actually “Need To Do”

First: define the words we teach with. Good clinical skills means - write it down. Bedside manner means - describe the behaviour. Academic atmosphere means - measure it. If you cannot define it, you cannot teach it, you cannot assess it, and you cannot be surprised that students are asking strangers on the internet to explain it.

Second: build real career counselling. Not inspirational. Not anecdotal. Data-driven, branch-wise, honest about gender, geography, capital, and global pathways. Students can handle truth. They cannot handle fog.

Third: retire the suffering mythology. Hard work is necessary. Needless suffering is lazy curriculum design calling itself tradition.

Fourth: treat social media as institutional feedback. Those WhatsApp screenshots, those Twitter threads, those midnight Reddit posts — they are what students cannot say inside classrooms. They are unsolicited feedback from people with nothing to gain by speaking. Read them.

Fifth: speak during institutional crises. When NEET is cancelled, when counselling collapses, when examination integrity fails, educator silence is not neutrality. It is abandonment. Students need someone to say: this is uncertain, this is unfair, here is what is known, here is what is not known, here is how to protect your preparation, here is where to find help. That is mentorship. “Don’t worry, focus on studies” is what adults say when they do not want to do adult work.

The Handshake, Finally Decoded

These phrases — good clinical skills, toxic department, lifestyle branch, noble profession, sacrifice phase, you will understand later, burnout is normal, merit is dead, connections matter, best branch, DNB vs MD, USMLE escape plan, saturation, first-gen disadvantage, academic atmosphere — are not just slang.

They are a generation trying to name things they can feel but were never given language for.

Underneath every phrase is one of the same small set of questions.

Will I be safe here? Will I be treated fairly? Will this profession give me meaning or only consume me? Will my teachers tell me the truth? Does anyone senior enough to know the answers care enough to say them out loud?

That is the real question.

And that is exactly where MedEd facilitators must enter.

Not as moral police. Not as nostalgic seniors who survived worse and want credit for it. Not as WhatsApp philosophers. Not as people who say “in our time also we suffered” as if suffering is a gift that must be passed forward.

But as translators. As witnesses. As the adults in the room.

The secret handshake spread because formal mentorship left a vacuum.

The answer is not to mock the handshake.

The answer is to build a room where students no longer need secret codes to say what they are actually living through.

Because Gen Z is not asking for softness. They are asking for clarity. And clarity is the one thing Indian medical education has been postponing for decades.

It is time to stop postponing….!

1. “Will machines ever think like human beings?”

• No. He said if the question is whether machines will ever think like human beings, he says no.

2. “Will they be more intelligent than human beings?”

• It depends on how you define intelligence. If the question is whether machines will ever be more intelligent than human beings, well, that depends on how you define intelligence.

His reasoning in the talk was characteristically practical, not mystical:

• Computers already beat us at arithmetic, “which computers dominate by their very nature — they don’t use their calculating apparatus in the same way as human beings use their brains”. You could build a machine to add slowly and make mistakes like we do, he joked, “but why would you want to?”

• In 1985 he still pointed to what humans did better: “the kind of pattern recognition required to recognize a single human face in different photographs”. That was before modern machine learning flipped that example.

• When pressed about machines discovering new ideas, he didn’t deny progress. He said: “I think that we are getting close to intelligent machines, but they’re showing the necessary weaknesses of intelligence”.

In short — Feynman didn’t see human-like thinking as the goal or the outcome. Machines would get alienly good at specific things, and whether that counts as “more intelligent” is a definitional fight, not a physics fact.

A Doctor attends a three-day medical conference. He has flown across a country, stayed in a hotel, sat through keynote lectures, collected accreditation points, renewed his professional certificate (not mandatorily), and returned to his clinic. By every official measure, he has been educated. The regulator is “satisfied”. The professional body is satisfied. The sponsor is very satisfied. The hotel is delighted.

Now consider the patient who arrived at that clinic the following Monday with a fever that had not subsided in seven days.

Was she safer?

The honest answer — the one that decades of educational research and a growing body of outcomes literature demand — is: we do not know. And the more troubling truth is that the systems governing physician education have been designed, perhaps unconsciously, to avoid finding out…!

A Market That Sells Certainty, Buys Comfort

The global continuing medical education industry is not a charitable enterprise. Estimates place its market value at approximately seven billion dollars annually, with projections pointing steadily upward as digital platforms layer onto existing conference infrastructure.¹ In countries with large physician workforces, the annual spend on medical meetings — national, regional, specialty, subspecialty, hybrid, and purely online — runs into figures that, if aggregated and audited honestly, would constitute one of the largest discretionary expenditures in the health sector.

This is not inherently wrong.

Medicine changes. Rapidly, unforgivingly, and in ways that cannot be learned once and remembered forever. The half-life of medical knowledge — the point at which half of what a doctor learned in training is either outdated or superseded — has been estimated at somewhere between two and five years for clinical disciplines.² A doctor who stops learning is not a stable practitioner. She is a slowly accumulating liability.

So the purpose of continuing education is beyond debate.

What is very much debatable and almost never debated with sufficient seriousness — is whether the architecture we have built to deliver it actually works….

Attendance Is Not Learning. Learning Is Not Competence. Competence Is Not Care.

Somewhere in the evolution of continuing professional development, a catastrophic shortcut was institutionalized: the hour of attendance became the unit of education.

Count the hours. Issue the certificate. Record the credit. Repeat annually. Call it a system.

This is not a system. It is a ritual of compliance that has been mistaken for a pipeline of improvement.

The distinction matters because the link between attendance and patient outcomes is not obvious — it is assumed. And that assumption has never been adequately tested. A landmark Cochrane review on educational meetings found that they can improve professional practice and healthcare outcomes, but that effects are highly variable and depend on the format, interactivity, baseline performance of participants, and the clinical complexity of the behaviour being targeted.³ The review’s most disquieting finding was buried in its methodology: across the literature, the most commonly measured outcome was physician self-report, not patient outcomes. Doctors were asked whether they had learned something. Patients were not asked whether anything had changed.

A 2024 scoping review examined 83 studies on continuing medical education conference evaluation published between 2008 and 2022. It found wide variation in evaluation methodology, no broadly applicable validated outcome tool across studies, and a pronounced tendency to assess attendee satisfaction rather than downstream clinical behaviour.⁴ Eighty-three studies. Fourteen years. No consensus instrument.

This is not a measurement gap. It is a measurement philosophy.

The philosophy says: we will measure what is easy to count, not what is important to know….!

The Room That Is Engineered

There is a conversation the medical profession needs to have with more honesty than it typically manages.

Much of continuing medical education is commercially supported. A significant (almost all) proportion of conference budgets, speaker travel, hospitality, exhibition spaces, satellite symposia, and printed materials are underwritten by entities with a commercial stake in physician prescribing, procedure selection, device adoption, and diagnostic ordering. This is not a secret. Every delegate understands the architecture as they walk past the branded stalls.

The debate usually collapses into two shallow positions. One side argues that commercial involvement contaminates everything. The other argues that without it, the educational infrastructure of medicine would collapse. Both are partially right, which means neither is fully useful…!

The more productive question is not whether commercial money shapes medical education. The evidence that it does is now substantial enough to be considered settled.⁵ The more productive question is how it shapes the room — not through overt corruption, which is rare and prosecutable, but through the quieter, more durable mechanisms of topic selection, speaker selection, framing of evidence, and the systematic exclusion of questions the market has no interest in funding.

Who sponsors a session on prescribing less?

Who funds a lecture on watchful waiting?

Who pays for a workshop on telling a patient that the most expensive intervention is sometimes the most unnecessary one?

The market loves escalation. Good medicine frequently demands restraint. That tension is not incidental to the problem. It is the problem.

Regulatory frameworks have begun to address the most visible forms of commercial influence. Codes governing pharmaceutical marketing practices in several jurisdictions now prohibit gifts, financial benefits, and hospitality to physicians and their immediate families, holding companies accountable for conduct throughout their promotional chains.⁶ These are necessary and welcome reforms. But regulation closes visible doors. It cannot build independent rooms. A code can stop the branded pen. It cannot redesign the curriculum….!

The Feedback Form That Asks the Wrong Question

The weakest ritual in medical education is the post-conference questionnaire.

Was the venue adequate? Was the speaker engaging? Was the content relevant? Would you attend next year?

These are reasonable questions for an event manager. They are almost entirely useless for a system that claims to be improving healthcare…!

Learner satisfaction and learning transfer are not the same variable. They are not even reliably correlated. A speaker can be riveting and change nothing. A session can feel directly relevant and leave no trace in practice three months later. The conference can feel transformative — and frequently does — while prescribing patterns, referral habits, diagnostic thresholds, and communication behaviours remain entirely undisturbed.

The educational sciences have known this for decades (the real irony). The Kirkpatrick model, developed for organisational learning in the 1950s, describes four levels of evaluation: reaction, learning, behaviour, and results.⁷ Medical education has operated almost exclusively at level one — reaction — for most of its history. This is not because levels three and four are unimportant. It is because they are hard to measure, require longitudinal follow-up, demand linkage to clinical data systems, and produce findings that may be uncomfortable.

A conference that produces no measurable behaviour change is not a failure of medicine. It is a failure of design. And a profession that applies rigorous evidence standards to the drugs it prescribes ought to apply at least the same rigour to the education it consumes.

The Patient Pays Twice

There is a patient who never enters the conference hall. She does not collect the delegate bag. She does not read the souvenir brochure. She does not know which satellite symposium was held in which hotel ballroom.

But she is not absent….!

When educational agendas are shaped by those who profit from clinical escalation, the clinical baseline quietly shifts. When physicians are repeatedly exposed to certain framings — certain molecules as defaults, certain devices as standards, certain combinations as modern — the concept of routine care inflates. The patient at the prescription counter pays the difference between evidence-based and commercially-inflated standard of care, often without any idea that the gap exists.

The World Health Organization has estimated that between 20 and 40 percent of health spending is wasted globally through inefficient care, including inappropriate prescribing.⁸ Irrational prescribing is not primarily produced by bad doctors. It is produced by environmental pressures — including educational environments — that normalize certain clinical choices while others quietly disappear from view.

This is where the ethical stakes of continuing medical education become a public health question, not merely a professional one. What physicians learn — and what they are systematically not taught — has consequences that cascade from the clinic to the household budget to the out-of-pocket spending that remains the leading cause of catastrophic financial hardship in low- and middle-income settings.

The Rupee That Never Reached the Patient

India’s out-of-pocket health expenditure stood at 39.4 percent of total health spending in 2021-22 — a hard-won decline from 64.2 percent in 2013-14, yet still among the highest proportions in any major economy, and one that pushes between three and seven percent of Indian households below the poverty line every year, with the burden falling most heavily on rural families and the poorest states.⁹

Against that backdrop, a question that Indian medicine has not yet seriously entertained deserves to be asked — loudly, repeatedly, and without the comfort of an easy answer.

• If the thousands of crores flowing annually through the medical education economy were even fractionally redirected — not to policy task forces, not to institutional infrastructure, not to yet another regulatory committee — but to the single most consequential gap between a sick Indian and a correct diagnosis, what might change?

• What if a mandatory portion of every accredited continuing medical education event were pooled into a district-level diagnostic support fund — subsidising the chest X-ray that a daily-wage labourer cannot afford, the neonatal screening that a rural family delays until it is too late, the advanced biochemical/microbiological test that converts a clinical suspicion into a confirmed pathogen and a rational antibiotic?

• What if professional accreditation of a conference depended not only on the quality of its scientific programme but on its demonstrable contribution to the investigative capacity of the population it claims to serve?

These are not comfortable questions, because they ask the profession to imagine a world in which resources that currently travel toward ballrooms and branded bags travel instead toward the patient who cannot reach either. They also invite the harder question that Indian medicine has historically preferred to leave unasked: when we speak of continuing medical education as a public good, are we describing a good that reaches the public — or one that circulates, lucratively and self-referentially, entirely within the profession itself..?

What Would Reform Actually Require?

Five changes (that i can currently think of) may transform this system. None of them are new. Several of them are overdue by decades.

• First, outcome linkage must replace attendance counting. Credit points for professional renewal should include some portion tied to evidence of practice reflection, not merely proof of presence. This does not require surveillance of individual physicians. It requires incentive design that rewards engagement with outcomes data — prescribing audits, case reviews, clinical indicators — rather than the passive accumulation of hours.

• Second, educational independence must be structural, not declarative. Conflict-of-interest disclosure, now widely required, is necessary but insufficient. It discloses; it does not neutralise. Scientific programme committees must be insulated from commercial teams by design, not by goodwill. Topics should be selected on the basis of documented care gaps — measured deficits in clinical practice — not on the basis of which sponsor is willing to fund the session.

• Third, the conference format must be challenged. The Cochrane evidence is clear: passive, didactic, large-group instruction is among the weakest formats for changing clinical behaviour.³ Interactive, case-based, problem-focused, multi-contact educational interventions outperform the classical plenary model by significant margins. The conference will survive this finding — physicians need community, debate, and professional fellowship — but it must be rebuilt around clinical impact rather than assembled around spectacle.

• Fourth, decentralisation must become policy. The geography of medical education in most countries remains skewed toward metropolitan/large centres. Moving knowledge to where patients live — through hybrid delivery, district-level workshops, simulation programmes in smaller hospitals — is not merely a question of equity. It is a question of epidemiological logic. The greatest care deficits are rarely where the most conferences are held.

• Fifth, evaluation must be prospective, not ceremonial. Before a major continuing education event, its designers should identify three to five measurable clinical gaps the programme aims to address. After the event — three months, six months — someone should check whether those gaps narrowed. The findings should be published. The failures should be learned from. The successes should be replicated.

This is not a utopian agenda. It is a minimum standard of intellectual honesty for a profession that holds itself to evidence…!

The Accountability That Medicine Owes Itself

The parallel is not unfair. Both make claims about improving patient outcomes. Both consume significant resources. Both carry risks of bias. One is subject to pre-registration, peer review, independent replication, and regulatory scrutiny. The other is subject to a satisfaction survey and a badge scanner.

This is not an argument against medical conferences. It is an argument that the profession’s commitment to evidence should extend to its own educational rituals.

The practice of medicine rests, at its foundation, on a social contract: the physician’s knowledge is continuously updated, fiercely independent, and ultimately in service of the patient’s interest. That contract is not honoured by attendance. It is honoured by change.

A conference that cannot trace a credible line from its stage to a patient’s outcome has not failed medicine. Medicine has failed to demand enough of it.

The patient was never in the hall. She was always the point…!!

And until she can feel the difference — in a more rational prescription, a safer procedure, a more honest conversation, a more independent clinical mind — we should stop calling what happens in those ballrooms education…..

We should call it what it is: a rehearsal….

A very expensive, very comfortable, very unchallenged rehearsal…….!!

I usually hesitate/decline.

Not because I dislike examining students, and not because I do not value assessment. I decline because examination halls now reveal more than most of us are prepared to admit. They do not merely show what students know. They show what the system has permitted them not to know…..!

This season, I said yes.

A mentor/well wisher asked me; one of the finest medical educators I know, a teacher whose judgment I trust more than most policy documents. His/Her institution was well regarded, with strong faculty, good clinical material, and the kind of academic tradition that still makes people believe excellence in Indian medical education is alive. I went and spent three days there. We examined approximately sixty undergraduate students.

I returned with a worry I cannot easily put away….!

Let me be precise at the outset. This is not a formal audit. It is not a statistical claim about one institution, one batch, or one generation. I am not naming the college, because the college is not the point. The institution has a strong reputation and many excellent teachers and that is precisely why the experience disturbed me. If these gaps appear in a well-regarded setting with adequate clinical exposure, then the problem is not merely local. It is cultural, systemic and is far more difficult to admit…!

More than sixty percent of the students I examined could not hold a knee hammer correctly…!

That number should stop us. Not interpret a complex reflex. Not distinguish a subtle upper motor neuron sign from a lower motor neuron lesion. Not localise a rare neurological pathology. Hold a knee hammer…! one of the earliest tools placed in a medical student’s hands, almost ceremonial in its simplicity. To hold it properly, to let the wrist move freely, to elicit a reflex with economy and confidence, is among the smallest acts of bedside medicine.

And yet, in that examination hall, it became evidence of something larger.

Some students held it like a pen. Some tapped without rhythm. Some had clearly memorised the sequence of neurological examination, they could recite it step by step — but the body betrayed the truth. They had seen the skill demonstrated. They had recorded the skill in logbooks. They had been signed off for the skill. They had not acquired it…..

The hand had not learnt what the logbook had certified.

That sentence formed itself somewhere in the second day and I have not been able to set it aside since.

The second shock was pharmacological. A significant number of students could not confidently tell me the correct dose of paracetamol….! not an obscure chemotherapy agent, not a rare metabolic drug, but the most ordinary medicine in the world, written daily in outpatient departments, fever clinics, emergency rooms, and pediatric wards across the country. When a final-year undergraduate cannot prescribe paracetamol with assurance, we are no longer discussing an academic gap. We are discussing unsafe training.

The third failure was harder to quantify but more worrying. Clinical reasoning was fragile. When asked to think through the ordinary presentations of everyday Indian practice; a febrile child, a child with cough, a child with seizures, a child not feeding well — students would often produce fragments: a definition here, a memorised list there, a few disconnected differentials. But the actual movement of clinical thought was absent. There was recall without understanding. Words without structure, confidence without competence and exposure without ownership.

One student, arriving on the third day, made the discomfort concrete….

He was polished, fluent, and relaxed — an NRI student at an institution whose name carries considerable social weight in Indian medical culture. Confidence is not the problem; medicine needs it. But confidence must be earned by competence and restrained by humility. This student had the outer structure of confidence without the inner burden of preparation. He seemed to regard the examination as a minor interruption, the examiners as peripheral figures, the institution’s reputation as a credential he had already inherited. My co-examiner and I noticed it within minutes. What followed was not impressive.

I do not want to make this essay about him, however. One arrogant student can be dismissed; a pattern cannot…..

The problem is not that some students are weak. Weakness can be remediated. A student who knows their gaps can be taught. A student who seeks correction can become competent. The more difficult problem is when weakness travels with entitlement — when a student is underprepared and also insulated, believing that the system is obligated to certify them regardless. That student is not merely a clinical risk. They are a systemic symptom…..

The disease is a training environment that has allowed students to graduate the identity of a competent doctor before earning the substance of one. A system where admission is understood — correctly, as it turns out — to be the real achievement, and everything thereafter is administrative. Where the entrance examination was the struggle, and clinical training is its extended, manageable aftermath.

We have a name for the ideology that created this environment. We call it student-friendliness, Learner-centredness, Mental health sensitivity….!

Some of it was necessary. Medical education did need to become less abusive. Humiliation is not pedagogy. Fear is not rigour. The old culture damaged many learners and confused intimidation with standards.

But in correcting one wrong, we may have created another. We have confused support with exemption. Compassion with lowered expectations. A safe learning environment with a consequence-free one. Student-centred education with student-immunised education. And we are now surprised when some students treat assessment as inconvenience rather than accountability…!

This is where Competency-Based Medical Education was supposed to intervene.

CBME was designed to ask a simple and honest question: can the student actually do what society needs a doctor to do? Can they examine a patient, prescribe safely, reason through a common complaint, recognise danger, and say — without shame — I do not know? It promised observable outcomes, structured feedback, formative correction, and competence before certification.

The idea was sound. The implementation, in too many places, has become clerical.

Logbooks are signed. Portfolios submitted. Competencies mapped. Modules attended. Internal assessments completed. Forms filed. Evidence generated. And at the bedside, the knee hammer is still held wrongly. At the prescription table, paracetamol dosage is still uncertain. At the viva table, clinical reasoning still collapses. We have become skilled at producing evidence of teaching. We are becoming worse at producing evidence of learning. These are not the same thing, and the gap between them is the distance between what CBME promised and what too many of us know it has delivered.

CBME now risks standing for Certificate-Based Medical Education. The acronym has survived the translation. The spirit has not…!

A signed logbook does not examine the patient. A completed portfolio does not calculate a dose. A mapped competency does not elicit a reflex. A certificate does not comfort a febrile child at midnight. Competence is not produced by paper. It is produced by repeated, observed, corrected practice — and that requires teachers who are present, students who are serious, institutions that protect standards, and examiners willing to fail a student when failure is the honest result.

That last point is now the most uncomfortable in medical education……

We have made failure almost unspeakable. To fail a student is to invite argument, paperwork, parental pressure, and institutional anxiety. Passing is easier. Borderline marks are easier. Quiet inflation is easier. Silence is easiest of all. But the consequences of that silence are not borne by the examiner. They are borne by patients and inturn the society at large…..!

The patient is the missing person in most of these debates.

We discuss students, faculty, universities, regulators, rankings, curriculum, logbooks, modules, and skills labs. The patient sits in an empty chair. Yet that empty chair is the moral centre of the room. The patient does not care whether the student is from a prestigious institution. They do not care whether the logbook is complete, whether the college ranks high, or whether the English is fluent. They care whether the doctor can think clearly, examine properly, prescribe safely, and act responsibly. Everything else is noise.

Prestige makes this worse in particular ways. Well-regarded institutions carry a special risk: they can begin to believe their own reputation. Students may absorb the atmosphere and conclude that admission was the proof of excellence, and training merely its administration. Faculty may assume the quality of intake guarantees the quality of output.

This is prestige as anesthesia — it numbs scrutiny, softens doubt, and hides incremental decay behind institutional legacy. Sometimes in a reputed institution, a bad performance becomes just an aberration, protected by myth.

But myths do not examine patients. Doctors do.

No system decays without cooperation. Some of that cooperation is active. Most of it is passive — the accumulated silence of people who saw, and adjusted, and moved on.

We sign logbooks without observing the skill. We inflate borderline marks to avoid conflict. We soften poor performance into “needs improvement.” We tell ourselves they will learn during internship. We ask ourselves what we can realistically do. We lower our eyes at the precise moment the system most needs us to raise them.

This is how standards die. Not by one grand collapse, but by a thousand small permissions. A wrong hammer grip, noted and ignored. A wrong dose, softened into approximation. A poor answer, inflated into a pass. A bad attitude, excused as nervousness. A dangerous gap, renamed as a learning need and deferred.

Eventually, the system learns to produce certificates that no longer mean what they claim.

This is not a call for cruelty. It is a call against dishonesty….

Medical education does not need to return to humiliation. Shouting teachers and frightened students are not rigour — they are failure of a different kind. But the answer to that failure is not indulgence. It is disciplined honesty. A student can be supported and still be failed. Respected and still be corrected. Protected from humiliation without being protected from standards. Heard without being permitted to negotiate competence. These distinctions are now urgent, because the world outside medical college will not negotiate.

A convulsing child will not wait for a remediation plan. A toxic prescription will not respect student anxiety. A missed diagnosis will not care about institutional prestige. A rural primary health centre will not provide multiple-choice options. The patient will ask the only question that matters: do you know what you are doing?

Our current system is too often answering that question with a certificate.

I am writing this for all the medical educators who have seen the same thing and remained quiet….

You have seen students who cannot examine. You have seen logbooks that mean nothing. You have seen fluency mistaken for competence, confidence mistaken for capability, institutional prestige mistaken for individual readiness. You have seen faculty lower standards because fighting every battle is exhausting. You have seen CBME become a documentation exercise. You have seen all of this, and you have said what I have often said — in the faculty room, over tea, in the car home — and then returned the next morning to a system that continued unchanged…..

The question is no longer whether we have seen it. The question is whether we are willing to say it.

Not in another polite seminar on implementation challenges. Not in another faculty development programme where everyone speaks the language of reform and returns to the habits of evasion. Not in another consultation that produces another checklist.

Publicly. Professionally. In our own names.

I went to that examination hall as a reluctant examiner. I left with that reluctance reinforced — and with something else that I think is more useful than reluctance.

Worry is different from cynicism. Cynicism has given up. Worry still believes something is worth saving….

I still believe medical education is worth saving. I still believe that a student can be transformed by real patients, honest feedback, repeated practice, and earned humility. I still believe CBME can mean competence.

But not if we keep signing what we have not seen. Not if we keep certifying what students cannot do. Not if we keep hiding decay behind legacy. Not if we keep confusing politeness with ethics, or paperwork with reform.

The knee hammer was a small thing. That is why it mattered.

Large systems reveal themselves through small failures. A hand that cannot hold a hammer correctly after years of clinical training is not merely a technical gap.

It is a message.

We should read it before the patient does.

The trial enrolled thousands of patients across major centres in Europe and North America. The participants were younger than the person in front of you, more closely monitored, and far easier to follow up. The primary endpoint was not always something a patient would recognize as recovery, but a ‘surrogate’ that moved in the right direction within the trial’s time frame. Your patient is older, poorer, more clinically entangled, and lives far from the hospital. She appears nowhere in the evidence. Her town does not appear on the trial map. Yet the recommendation reaches her with full authority….

This is not an exception. This is modern “Evidence Based medicine”. This is not a failure of the individual trial. It is the ordinary output of a system that has learned to produce certainty for the populations it finds convenient, and export that certainty to everyone else.

The randomized trial remains the most powerful instrument of causal inference that medicine has produced. But the industry that now funds, organizes, and circulates randomized trials has bent that instrument toward questions that are profitable, and away from the populations that carry the greatest burden of illness. For clinicians in low and middle income countries, understanding the difference between the method and the industry around it is no longer optional. It is a form of self-defense…!

How an Agricultural Idea Became Medicine’s Cleanest Method

Ronald Fisher did not invent randomization to cure anything. In the 1920s, at Rothamsted agricultural station in Hertfordshire, he was trying to grow better wheat, and his problem was invisible variation — ‘in soil, drainage, and sunlight’ — that would confound any honest comparison of fertilizers unless it was distributed by chance. Randomization was not a method of finding truth. It was a discipline against pretending you already had it.

Austin Bradford Hill brought that discipline into medicine in 1948, in the streptomycin trial for pulmonary tuberculosis. Streptomycin was scarce, tuberculosis was killing people, and clinicians were certain the drug worked. Hill insisted on sealed envelopes anyway. Chance would decide who received treatment — not intuition, not seniority, not the attending physician's conviction.

Allocation by chance replaced conviction. Medicine acquired not just a method, but a moral position: when we do not know, we must not pretend that we do.

That was the brilliance of the randomized trial. It did not begin as an assertion of certainty. It began as an admission of uncertainty…!

The trouble is that the system built around the method no longer rewards that humility.

What the Industry Optimizes For

By the late twentieth century, the randomized trial had become more than a method. It had become infrastructure, regulation, and market strategy. Pharmaceutical companies came to fund most large trials. Contract research organizations industrialized the process. Protocol writing, site activation, data cleaning, regulatory filing, all could now be bought, scaled, and delivered.

This changed what trials are asked to do.

A publicly funded trial begins, ideally, by asking what is most urgently unknown. An industry trial begins by asking what evidence is needed to satisfy a regulator. These are not the same question, and the gap between them shapes the entire evidentiary landscape on which clinicians practice.

It is why medicine has hundreds of high-quality trials comparing patentable compounds to each other, and far fewer asking whether a community health worker, a mobile phone reminder, or an improved diagnostic pathway prevents death in a low-resource setting. The first set of questions can generate revenue. The second almost never can. The consequence is not a knowledge gap at the margins — it is a structural deformation of what gets studied, what gets published, and what eventually gets practiced…!

“Surrogate endpoints” are the instrument of this deformation. Procalcitonin, IL-6, HbA1c, LDL, ejection fraction, tumour shrinkage — these outcomes are faster to capture, cheaper (relatively) to follow, and often sufficient for regulatory approval. They are also several steps removed from what patients actually experience. The distance between what a trial measures and what a patient undergoes — what researchers call the efficacy-effectiveness gap — grows each time a proxy is accepted for an outcome, and a proxy is accepted for a patient…

That gap is not evenly distributed. It falls hardest on populations that were never in the trial to begin with…!

The Clean-Patient Problem, and Why Pediatrics Has It Worst

Industry trials prefer the kind of patients who make trials efficient: younger, better adherent, single-diagnosis, accessible for fortnightly follow-up, stable on fewer drugs. These patients move quickly through ethical approval and data collection. They are not, in any meaningful sense, the patients most of us treat.

Older adults are underrepresented. Pregnant women are routinely excluded. Multimorbid patients are filtered out in practice, even when not explicitly excluded on paper. Negative findings vanish more easily when sponsors own both the question and the data.

In pediatrics, this distortion is often more severe and less acknowledged. Children enter the trial literature late, thinly, and unevenly. They are harder to recruit, ethically more complex to enroll, and commercially less attractive to sponsors. A neonate is not a toddler. A toddler is not a school-aged child. A school-aged child is not an adolescent. Yet adult evidence routinely travels into pediatric practice through off-label prescription, dose extrapolation, and the quiet professional understanding that “there is no pediatric data” does not prevent clinical necessity. Structured uncertainty becomes routine without ever being called what it is.

Even when pediatric trials exist, they often satisfy regulatory minimalism rather than clinical depth — narrow eligibility, small numbers, short endpoints. Better than nothing, but rarely enough to answer what the bedside actually requires.

In a pediatric ward in a low-income setting, the gap is wider still. The children who fill those cots are rarely the children in pristine datasets. They arrive with prematurity, undernutrition, prior irrational antibiotic exposure, incomplete vaccination, late referral, and interrupted drug access. The guideline arrives with the same confidence regardless. The conditions it was built for do not travel with it.

Geography Is Not a Footnote

The problem of LMIC populations in clinical trials is typically framed as a diversity gap — a matter of incomplete enrollment, of demographic checkboxes not fully filled. That framing is inadequate, because it implies the solution is numerical.

The deeper problem is structural. More than eighty percent of the global burden of disease is carried by people in low- and middle-income countries. The research agenda is shaped largely elsewhere. LMIC populations appear in trial site lists — often because their patients are accessible, their regulatory timelines faster, and their per-patient costs lower — but the question was designed elsewhere, the authorship belongs elsewhere, and the conclusions return to LMIC clinicians dressed as universal truth. Their populations appear in site lists, often as recruitment pools rather than intellectual partners. Their physicians serve as sub-investigators.

This is not under-representation. It is extraction: the removal of biological and epidemiological value from a population without returning ownership of the knowledge produced.

The point needs to be made plainly because polite language often blunts it. The problem is not merely that our populations are insufficiently enrolled. The problem is that they are often enrolled in order to answer someone else’s regulatory question, under someone else’s assumptions, for someone else’s market…!

The Regulatory Loop That Completes the Circuit

This matters enormously in India.

In India, the mechanism by which imported certainty becomes clinical practice runs through the regulatory pathway. For decades, drugs entered Indian practice primarily on the strength of data approved by the FDA or EMA — data generated in populations that did not reflect Indian pharmacogenomics, Indian comorbidity profiles, or Indian treatment realities. When SGLT2 inhibitors were incorporated into international heart failure guidelines as Class I recommendations, South Asian enrollment in the pivotal trials was in the single digits as a percentage. The monitoring protocols those trials required were unavailable in most Indian district hospitals. The drug cost remained prohibitive for the majority of the target population.

None of this prevented the recommendation from arriving with full authority, because regulators deferred to the approval, guidelines deferred to the trial, and clinicians were expected to defer to the guidelines.

This is where evidence-based medicine becomes most fragile — not when it lacks trials, but when it stops asking the one question that should precede every recommendation: evidence about whom, produced under what conditions, and what is lost in the translation to here?

That question should be routine and not optional.

The Method Still Deserves Defense

None of this means the randomized trial should be discarded. That would be foolish. The randomized trial, when designed around a public question, can still do extraordinary things.

RECOVERY is probably the clearest demonstration. A government-funded trial embedded in ordinary NHS hospitals, asking which drugs actually worked against COVID-19 in the wards where people were dying. Its protocol fit on two pages. Its patients were unremarkable — old, young, complicated, straightforward — because the question was unremarkable: does this work, in ordinary care, for ordinary patients? It found that dexamethasone, a decades-old steroid costing pence per dose, reduced mortality by a third. It found that hydroxychloroquine did not work, and stopped using it without waiting for the publication cycle to complete. It is estimated to have saved over a million lives at a total trial cost of approximately two million pounds.

RECOVERY worked because the question was unowned. No company needed the answer for a regulatory filing. The method, freed from the commercial apparatus, returned to what it was always capable of — disciplined, humble, consequential.

The ANTICOV trial made the same point from African soil: African investigators, African health system infrastructure, asking what early COVID treatments actually mattered for patients African health systems were trying to serve. The trials that serve populations are, it turns out, the trials designed with those populations in mind from the first sentence of the protocol.

The Question That Should Precede Every Guideline

Fisher gave us randomization to stop us fooling ourselves about fertilizer. Hill gave medicine a method that treated uncertainty as a discipline rather than an embarrassment. The method has not failed. The economy built around it has learned to produce certainty selectively — for the populations it finds convenient, through endpoints it finds tractable, in directions it finds profitable.

Every landmark trial, every confident guideline, every Class I recommendation carries within it an answer to a question that rarely appears on the label: whose disease was studied, by whom, under what conditions of monitoring and access, and what happens when those conditions do not travel?

In a district hospital in Karnataka, on a Tuesday morning, a child is waiting. She is four years old. She is not in the trial. She is in your clinic. The guideline is confident.

Whether to apply it, adapt it, or interrogate it is clinical judgment. But clinical judgment cannot be exercised honestly without knowing what the evidence was actually built for.

Until that knowledge becomes standard practice — embedded in our training, demanded by our guideline panels, enforced by our journals — too much of what we call evidence-based medicine will remain what it quietly already is: borrowed certainty, applied to strangers, with the borrowing nowhere acknowledged.

That is the distinction the profession keeps forgetting…!

And until we remember it, especially in pediatrics and especially in low-resource settings, too much of what we call evidence-based care will remain what it already is: a guess, dressed in better language…!
